2种麻醉方式对中老年下肢静脉曲张手术患者睡眠质量影响的研究

Effect of Two Anesthesia Methods on Sleep Quality after Varicose Vein Surgery of Lower Extremity in Middle-aged and Elderly Patients

目的:观察分析2种不同麻醉方法对老年下肢静脉曲张手术患者术后睡眠质量的影响。方法:选取2018年5月至2019年5月邹平市人民医院收治的老年下肢静脉曲张需手术治疗的患者60例作为研究对象,按照排列分组法随机分为观察组和对照组,每组30例。观察组使用0.375%罗哌卡因和0.4μg/kg右美托咪定混合液50 mL麻醉;对照组使用0.375%罗哌卡因和生理盐水50 mL麻醉。观察2组患者的手术麻醉效果及手术后的睡眠质量。结果:术后,2组患者睡眠质量、入睡时间、睡眠效率、睡眠障碍评分均改善,且观察组患者的睡眠质量评分显著优于对照组患者。2组比较差异均有统计学意义(均P<0.05)。麻醉后,观察组拔管时间、睁眼时间、第1次给予镇痛药时间均短于对照组,2组比较差异均有统计学意义(均P<0.05)。麻醉后,2组患者血压和心率均降低;拔管后,2组患者血压均升高,2组比较差异均有统计学意义(均P<0.05)。结论:右美托咪定在老年下肢静脉曲张患者手术麻醉中应用临床效果较好,能够有效提升患者的睡眠质量。

Objective:To observe and analyze the effects of two different anesthesia methods on postoperative sleep quality of elderly patients with varicose veins of lower limbs.Methods:A total of 60 cases of elderly patients with varicose veins of lower limbs were selected.According to the arrangement and grouping method,they were randomly divided into observation group and control group,with 30 cases in each group.The observation group used 0.375%ropivacaine and 0.4μg/kg dexmedetomidine mixed solution 50 mL.The control group used 0.375%ropivacaine and physiological saline 50 mL.The anesthetic effect and sleep quality after operation were observed in the two groups.Results:After operation,the sleep quality,sleep time,sleep efficiency and sleep disorder score of the two groups were improved,and the sleep quality score of the observation group was significantly better than that of the control group.There were significant differences between the two groups(Ps<0.05).After anesthesia,the extubation time,eye opening time and the first administration of analgesics in the observation group were shorter than those in the control group.There were significant differences between the two groups(Ps<0.05).Blood pressure and heart rate decreased in both groups after anesthesia.After extubation,the blood pressure of the two groups increased,and the difference between the two groups was statistically significant(Ps<0.05).Conclusion:Dexmedetomidine has a good clinical effect in the surgical anesthesia of elderly patients with lower extremity varicose veins,and can effectively improve the sleep quality of patients.
