桑杏汤加减联合阿奇霉素对肺炎支原体感染患儿的疗效

Curative effect of modified Sangxing Decoction combined with azithromycin on mycoplasma pneumonia infection in children

目的探讨口服桑杏汤加减联合阿奇霉素治疗肺炎支原体(MP)感染患儿的临床疗效及对血清白细胞介素-6(IL-6)和降钙素原(PCT)的影响。方法选取80例MP感染患儿,采用随机数字表法分为2组,对照组40例采用阿奇霉素治疗,试验组40例在对照组治疗的基础上联合桑杏汤加减口服,对比2组中医主症候评分、T淋巴细胞亚群、症状及体征消失时间、炎症指标水平以及不良反应发生率。结果治疗5 d,试验组中医主症候评分显著高于对照组(P<0.05)。治疗10 d,试验组CD4+、CD4+/CD8+、肺活量(VC)、第一秒用力呼气容积(FEV_(1))和肺一氧化碳弥散量(DLCO)均显著高于对照组(P<0.05),CD8+显著低于对照组(P<0.05)。试验组血清降钙素原(PCT)、白细胞介素-6(IL-6)和C-反应蛋白(CRP)水平显著低于对照组(P<0.05)。治疗期间,试验组肺部啰音消失时间、退热时间和止咳时间显著短于对照组(P<0.05)。结论在阿奇霉素治疗MP感染患儿的基础上,联合桑杏汤加减治疗,可显著缩短肺炎症状及体征消失时间,抑制炎症反应,提升其呼吸功能和肺功能。

Objective To explore the clinical curative effect of oral modified Sangxing Decoction combined with azithromycin on mycoplasma pneumoniae(MP)infection in children and its influences on serum interleukin-6(IL-6)and procalcitonin(PCT).Methods A total of 80 children with MP infection were enrolled and divided into control group(40 cases,given azithromycin treatment)and experimental group(40 cases,orally given modified Sangxing Decoction on the basis of control group)by random number table method.The Traditional Chinese Medicine(TCM)main symptom score,T lymphocyte subsets,respiratory function indexes,disappearance time of symptoms and signs,levels of inflammation indexes and incidence of adverse reactions were compared between the 2 groups.Results After 5 days treatment,the TCM main symptom score were significantly higher than those in control group(P<0.05).After 10 days treatment,the CD4+,CD4+/CD8+,vital capacity(VC),forced expiratory volume in 1 second(FEV_(1))and carbon monoxide diffusion capacity(DLCO)in experimental group were significantly higher than those in control group(P<0.05),the CD8+were significantly lower in experimental group than control group(P<0.05).The levels of serum PCT,IL-6 and C-reactive protein(CRP)were significantly lower in experimental group than control group(P<0.05).During treatment,the antipyretic time,cough relieving time and disappearance time of lung rales in experimental group were significantly shorter than those in control group(P<0.05).Conclusion Azithromycin combined with modified Sangxing Decoction can significantly shorten the disappearance time of pneumonia symptoms and signs,inhibit inflammation,improve respiratory function and lung function in children with MP infection.
