摘要
目的探讨中性粒细胞与淋巴细胞比值(NLR)联合血小板与淋巴细胞比值(PLR)早期预测急性胰腺炎(AP)严重程度的价值。方法收集2019年6月~2021年7月本院收治的急性胰腺炎患者82例。按照疾病分类标准将急性胰腺炎患者分SAP组(n=40)即重症急性胰腺炎和非SAP组(n=42)即未达到SAP标准的AP。比较两组临床资料和生化指标;采用Spearman秩相关检验分析NLR、PLR与Ranson评分和APACHE II的相关性。应用ROC曲线分析NLR联合PLR对于SAP的预测效能。结果两组WBC、LYM、PLT、NEUT、BUN、NLR、PLR、Ranson评分和APACHE II评分比较,差异有统计学意义(P<0.05);相关性分析结果显示:NLR与Ranson评分、APACHE II评分呈正相关(P<0.05),PLR与Ranson评分呈正相关(P<0.05);ROC分析结果显示:相较于NLR、PLR,两者联合对于SAP预测效能较高,AUC为0.880,灵敏度为67.65%,特异度为88.24%(95%CI:0.804~0.957,P<0.05)。结论将NLR与PLR联合使用对SAP患者病情严重程度的预测价值更高。
Objective To investigate the early predictive value of neutrophil to lymphocyte ratio(NLR)combined with platelet to lymphocyte ratio(PLR)in the severity of severe acute pancreatitis(SAP).Methods A retrospective analysis method was used to collect clinical data of 82 patients with acute pancreatitis admitted to our hospital from June 2019 to July 2021.According to the newly revised classification criteria of Atlanta in 2012,patients with acute pancreatitis were divided into SAP group(n=40)and non-SAP group(n=42).The clinical data and biochemical indicators of the two groups were compared,and the Spearman rank correlation test was used to analyze the correlation between NLR,PLR and Ranson score and APACHE II.The ROC curve was used to analyze the predictive performance of NLR combined with PLR for SAP.Results There was a statistically significant difference between the two groups of WBC,LYM,PLT,NEUT,BUN,NLR,PLR,Ranson score and APACHE II score(P<0.05).The results of correlation analysis showed that NLR was positively correlated with Ranson score and APACHE II score(P<0.05),and PLR was positively correlated with Ranson score(P<0.05).ROC analysis results show that compared with NLR and PLR,the combination of the two has higher predictive power for SAP,with AUC of 0.880,sensitivity of 67.65%,and specificity of 88.24%(95%CI:0.804~0.957,P<0.05).Conclusion Compared with the use of NLR and PLR alone,the combination of NLR and PLR has a higher predictive value for the severity of SAP patients.
