非可视化椎间孔成形技术在侧路椎间孔镜手术中的临床应用 被引量:1

摘要 目的观察非可视化椎间孔成形技术在经皮椎间孔内镜椎间盘切除术(PETD)中的临床疗效。方法选取腰椎间盘突出行PETD的患者79例,术中采用非可视化椎间孔形技术,统计该组患者的手术时间、术中透视次数及术后并发症情况。采用疼痛视觉模拟评分(visual analogue scale,VAS)、OSWESTRY功能障碍指数(oswestry disability index,ODI)评分和改良MacNab疗效评定标准评价治疗效果。结果所有患者手术顺利,手术时间平均(62.10±19.32)min,术中透视次数平均为(8.0±3.5)次。无严重神经损伤、出血、感染等并发症。随访时间6~18个月(平均10.5个月)。术后各时间点VAS评分及ODI评分较术前均有显著改善(P<0.05);术后6个月随访MacNab评分65例优,10例良,3例可,1例差,优良率为94.9%。结论PETD采用非可视化椎间孔成形侧路椎间孔镜手术治疗腰椎椎间盘突出症,具有安全有效、步骤简化,透视少等优点,值得临床推广应用。
