慢性阻塞性肺疾病患者血浆丙烯醛的浓度与肺功能的相关性研究

摘要 目的香烟烟雾及油烟雾中存在高浓度的丙烯醛,会诱发慢性阻塞性肺病(COPD)的发病,本研究旨在探索COPD患者外周血中血浆丙烯醛浓度及其与肺功能的关系,为COPD发生发展的病因提供新的研究方向。方法选取2020年11月1日至2021年5月12日就诊于佳木斯大学附属第一医院呼吸与危重症医学科门诊及住院部的COPD患者118例,按照肺功能GOLD分级分为GOLDⅠ~Ⅳ级,分别记录各组患者的香烟烟雾及油烟雾接触史,检测其血浆丙烯醛浓度及肺功能。结果COPD患者血浆丙烯醛浓度与FEV_(1)占预计值百分比(FEV_(1)%)及FEV_(1)/FVC呈负相关(r=-0.898,r=-0.859),差异有统计学意义(P<0.05)。香烟烟雾接触史与FEV_(1)/FVC呈负相关(r=-0.792),与FEV_(1)%呈负相关(r=-0.826);油烟雾接触史与FEV_(1)/FVC呈负相关(r=-0.388),与FEV_(1)%呈负相关(r=-0.335),差异有统计学意义(P<0.05)。结论COPD患者香烟烟雾及油烟雾接触时间越长,血浆丙烯醛浓度越高,FEV_(1)%、FEV_(1)/FVC越低,肺功能损害越严重。 Objective A high concentration of acrolein in cigarette smoke and oil smoke will induce chronic obstructive pulmonary disease(COPD).The study was intended to explore the relationship between plasma acrolein concentration in peripheral blood and pulmonary function in COPD patients,and to provide a new research direction for the etiology of the occurrence and development of COPD.Methods A total of 118 COPD patients visiting the outpatient and inpatient departments of the Department of Respiratory and Critical Care Medicine of the First Affiliated Hospital of Jiamusi University from November 1,2020,to May 12,2021,were enrolled and classified into GOLDⅠ-Ⅳaccording to the GOLD stages of lung function.The history of cigarette smoke and oil smoke exposure in each group was recorded respectively,and their plasma acrolein concentration and lung function were measured.Results The plasma acrolein concentration of COPD patients was negatively correlated with the per centage of FEV_(1) in the predicted value(F EV_(1)%)and FEV_(1)/FVC(r=-0.898,r=-0.859),with statistically significant differences(P<0.05).The history of cigarette smoke exposure was negatively correlated with FEV_(1)/FVC(r=-0.792)and FEV_(1)%(r=-0.826),and the history of oil smoke exposure was negatively correlated with FEV_(1)/FVC(r=-0.388)and FEV_(1)%(r=-0.335),with statistically significant differences(P<0.05).Conclusion The longer the cigarette smoke exposure and oil smoke exposure,the higher the plasma acrolein concentration,the lower FEV_(1)%and FEV_(1)/FVC,and the more serious the damage of pulmonary function in COPD patients.
