不同他汀类药物联合西洛他唑对糖尿病周围血管病变患者炎症因子和血液流变学的影响比较

Comparison of the effects of different statins combined with cilostazol on inflammatory factors and hemorheology in patients with diabetic peripheral vascular disease

目的分析不同他汀类药物联合西洛他唑对糖尿病周围血管病变患者炎症因子和血液流变学的影响。方法经医院伦理学会批准,选择医院2019年1月1日~2020年1月1日诊治的2型糖尿病周围血管病变86例患者作为治疗对象。采用随机数表法将其分成对照组和研究组,各43例。对照组患者使用瑞舒伐他汀联合西洛他唑治疗,研究组患者使用匹伐他汀联合西洛他唑治疗。比较两组患者炎症因子、血液流变学、临床疗效及不良反应。结果治疗前两组患者血清高敏C反应蛋白(hs-CRP)水平比较差异无统计学意义(P>0.05)。治疗后研究组患者hs-CRP(4.18±0.69)ng/ml低于对照组(P<0.05)。治疗前两组患者血浆黏度及红细胞聚集指数差异比较无统计学意义(P>0.05)。治疗后研究组患者血浆黏度(0.87±0.19)mPa·s及红细胞聚集指数(3.03±0.19)低于对照组(P<0.05)。研究组患者临床总有效率高于对照组(P<0.05)。研究组患者不良反应总发生率低于对照组(P<0.05)。结论匹伐他汀联合西洛他唑治疗2型糖尿病周围血管病变能获得更好的临床疗效,可以显著抑制炎症反应,改善血液流变学。

Objective To analyze the effects of different statins treatment combined with cilostazol on inflammatory factors and hemorheology in patients with diabetic peripheral vascular disease.Methods With the approval of the hospital ethics committee,86 type-2 diabetic patients with peripheral vascular disease diagnosed and treated in our hospital from January 1,2019 to January 1,2020 were selected,and randomly divided into two groups by random number table,each with 43 cases.Control group received rosuvastatin and cilostazol,while study group received pitavastatin combined with cilostazol.Then the inflammatory factors,hemorheology,clinical efficacy and adverse effect rate were compared between the two groups.Results Level of serum high-sensitivity C-reactive protein(hs-CRP)yielded no significant difference between two groups at the baseline(P>0.05),while the hs-CRP level was(4.18±0.69)ng/ml in study group,which was significantly lower than that of control group after treatment(P<0.05).The plasma viscosity(PV)and erythrocyte aggregation(EA)demonstrated no significant difference between two groups before treatment(P>0.05),while the two were(0.87±0.19)mPa·s and(3.03±0.19)in study group after treatment,which were significantly lower than those of control group(P<0.05).The overall efficacy rate of study group was significantly higher than that of control group(P<0.05).The adverse reaction rate was lower in study group than in control group(P<0.05).Conclusion Application of pitavastatin combined with cilostazol for type-2 diabetic patients with peripheral vascular disease can effectively attenuate the inflammation reaction and improve the hemorheology.
