空心加压螺钉治疗股骨颈骨折

The cannulated compression screws foRtreatment of femoral neck fractures

目的探讨空心加压螺钉治疗股骨颈骨折的临床疗效。方法采用空心加压螺钉治疗56例股骨颈骨折患者。观察骨折复位质量、骨折愈合及并发症发生情况。结果患者均获得随访,时间6~60个月。术后6个月56例骨折复位均满意;49例随访7~12个月,复位质量满意,其中骨折不愈合2例,股骨头坏死2例;31例随访36~60个月,复位质量满意,其中骨折不愈合1例,股骨头坏死3例,髋内翻伴螺钉切割穿出1例。结论空心加压螺钉治疗股骨颈骨折稳定性好、并发症少,但是需要有效的复位、可靠的固定和良好的术后管理。

Objective To investigate the clinical effect of cannulated compression screws in the treatment of femoral neck fractures.Methods The 56 patients with femoral neck fractures were treated with cannulated compression screws.The quality of fracture reduction,fracture healing and complications were observed.Results All patients were followed up foR6~60 months.At 6 months postoperation,56 cases were all satisfactory with the good fracture reduction;49 patients were followed up foR7~12 months,and the quality of reduction was satisfactory,including 2 cases of nonunion,and 2 cases of femoral head necrosis;31 patients were followed up foR36~60 months,the quality of reduction was satisfactory,of which 1 case was nonunion,3 cases had femoral head necrosis,and 1 case had hip varus with screw cutting and penetration.Conclusions The cannulated compression screw has good stability and few complications in the treatment of femoral neck fractures,but it needs effective reduction,reliable fixation and good postoperative management.
