院前院内一体化急救模式应用于急性脑梗死中的价值评价

Evaluation value of prehospital and hospital integrated emergency mode in acute cerebral infarction

目的:讨院前院内一体化急救模式应用于急性脑梗死中的价值。方法:采用院前院内一体化模式救治的64例脑梗死患者为观察组,按照1:1选取同期由64例家属护送采用常规院内急救模式救治的急性脑梗死患者为观察组,比较两组急救时间、NIHSS评分及Barthe指数。结果:观察组转运时间(22.10±6.67)min、确诊时间(34.60±9.45)min、确诊至溶栓时间(73.45±14.65)min、急救总时间(130.10±22.56)min、全部短于对照组的(38.23±8.65)min、(40.76±10.03)min、(97.80±16.48)min、(182.34±29.34)min(P<0.05)。就诊时两组NIHSS、Barthe无差异(P<0.05),溶栓后1周观察组NIHSS(8.31±2.87)低于对照组的(10.42±2.98),差异显著(P<0.05),溶栓后3月Barthe观察组(72.96±8.56)高于对照组的(66.85±7.82),差异显著(P<0.05)。结论:无缝隙院前院内一体化急救模式的有效运用,可实现急性脑梗死转运、确诊、溶栓时间,降低神经功能缺损程度,提高日常生活能力,值得应用。

Objective:To discuss value of prehospital and hospital integrated emergency mode in acute cerebral infarction.Method:cerebral infarction patients 64 cases treated by pre hospital and hospital integrated mode were selected as observation group,and 64 patients with acute cerebral infarction treated by routine hospital emergency mode escorted by family members were selected as t observation group according to 1:1.The first aid time,NIHSS score and Barthe index were compared between the two groups.Result:The observation group transport time of(22.10±6.67)min,diagnosis(34.60±9.45)min,from diagnosis to thrombolysis(73.45±14.65)min,total time of first aid(130.10±56)min,all shorter than control group(38.23±8.65)min、(40.76±10.03)min、(97.80±16.48)min、(182.34±29.34)min(P<0.05).There was no difference in NIHSS and Barthe between two groups(P<0.05).One week after thrombolysis,NIHSS in the observation group(8.311±87)was lower than control group(10.42±2.98)(P<0.05).3 months after thrombolysis,Barthe(72.96±8.56)was higher than control group(66.85±7.82)(P<0.05).Conclusion:The effective application of seamless pre hospital and in hospital integrated emergency mode can achieve the time of transportation,diagnosis and thrombolysis of acute cerebral infarction,reduce the degree of neurological deficit,and improve the ability of daily life,which is worthy of application.
