摘要
目的:评估基于多媒体的术前护理访视对胸腹腔镜食管癌患者围术期的抑郁状况及血清皮质醇的影响。方法:80例行胸腹腔镜食管癌手术治疗的患者按随机数字表法分为观察组和对照组,各40例。观察组采用结合多媒体形式的术前访视,除口头宣教外,播放一段约15 min的宣教视频。对照组采用常规以口头宣教为主的术前访视。评估两组患者围术期抑郁状况及血清皮质醇浓度。结果:两组患者入院后次日抑郁评分、抑郁症状程度及血清皮质醇水平差异均无统计学意义(均P>0.05)。术前1 h和术后24 h观察组患者抑郁评分、抑郁症状程度及血清皮质醇水平均显著低于对照组(均P<0.05)。结论:多媒体术前访视能有效降低胸腹腔镜食管癌患者围术期的心理应激反应。
Objective:To evaluate the effects of preoperative multimedia-based nursing visit on perioperative depression and serum cortisol in esophageal cancer patients undergoing video-assisted thoracoscopic surgery.Methods:According to random number table method,a total of 80 patients were randomly divided into observation group and control group,40 cases each group.Multimedia-based preoperative visit was used in the observation group.In addition to oral education,a video of about 15 minutes was played.In the control group,the routine preoperative visit was oral education.The perioperative depression level and serum cortisol was evaluated in two groups.Results:There was no statistically significant difference in baseline depression score,depressive symptoms and vital serum cortisol level between two groups(all P>0.05).At one hour before surgery and twenty-four hours after surgery,compared with control group,the observation group reported significant lower depression score,depressive symptoms and serum cortisol level(all P<0.05).Conclusion:Multimedia-based preoperative nursing visit could effectively prevent the psychological stress response in esophageal cancer patients undergoing video-assisted thoracoscopic surgery.
