基于扎根理论构建中青年脑卒中高血压护理团队管理模型的质性研究

Teamworking model of nursing young and middle-agedpeople with stroke and hypertension:A ground theory study

目的探索中青年脑卒中患者高血压护理团队要素,构建其护理团队模型。方法采用质性研究中扎根理论的方法,对15名临床护士及护理管理者进行半结构式个体访谈,运用Corbin和Strauss三级编码及持续比较的方法进行分析和编码。结果中青年脑卒中患者高血压护理团队模型可归纳为三个主题:团队核心成员(多学科专家、专科护士和患者)、团队主要职能(科研循证、教育培训、远程护理)、团队运作流程(计划、实施、检查、分析)构成。同时提出4个理论假设。结论中青年脑卒中高血压护理团队模型有助于护士职业发展、能发挥多学科团队协同作用、促进中青年脑卒中高血压护理团队运作效能等特点,对中青年脑卒中患者高血压管理的理论研究和临床实践具有指导意义。

Objective To explore the factors of nursing team for middle-aged and young stroke patients with hypertension and construct their nursing team model.Methods Using the method of grounded theory in qualitative research,15 clinical nurses and nursing managers were interviewed by semi-structured individuals.Corbin and Strauss three-level coding and continuous comparison were performed to analyze and code.Results The model of hypertension nursing team for middle-aged and young stroke patients could be summarized into three themes:the core members of the team(multidisciplinary experts,specialized nurses and patients),the main functions of the team(scientific research evidence-based,education and training,remote nursing),and the operation process of the team(planning,do,check,analysis).At the same time,four theoretical hypotheses were proposed.Conclusion The nursing team model of stroke and hypertension in young and middle-aged patients might be helpful for the career development of nurses,and play the role of multidisciplinary team synergy,as well as promote the operation efficiency of nursing team of stroke and hypertension in young and middle-aged patients,which have the guiding significance for theoretical research and clinical practice of hypertension management in young and middle-aged patients with stroke.
