不同入路单孔腹腔镜子宫切除术治疗子宫良性疾病的疗效分析

Curative effect analysis of different approaches in single incision laparoscopic hysterectomy for benign uterine diseases

目的:探讨单切口腹腔镜手术、专用入路及手套入路单孔腹腔镜子宫切除术治疗子宫良性疾病的临床疗效。方法:回顾分析2018年1月至2020年9月收治的行子宫切除术的117例子宫良性疾病患者的临床资料,其中48例行专用入路单孔腹腔镜手术(A组),36例行单切口腹腔镜手术(B组),33例行手套入路单孔腹腔镜手术(C组);比较三组术前临床资料及术中、术后指标。结果:3组患者术前临床资料差异无统计学意义(P>0.05);B组切口长度长于A组、C组,差异有统计学意义(P<0.05);B组、C组手术时间长于A组,差异有统计学意义(P<0.05);A组、C组美容评分高于B组(P<0.05);A组美容评分高于C组,差异有统计学意义(P<0.05)。结论:单切口腹腔镜手术、专用入路及手套入路单孔腹腔镜手术治疗子宫良性疾病各有优缺点,可根据患者病情、经济条件及对美容的要求合理选择。

Objective:To investigate the clinical effect of single port laparoscopic surgery,special approach and glove approach single port laparoscopic hysterectomy in the treatment of benign uterine diseases.Methods:Clinical data of 117 patients with benign uterine diseases who underwent hysterectomy were retrospectively analyzed in the period from Jan.2018 to Sep.2020,including 48 cases with special approach single port laparoscopic surgery(group A),36 cases with single incision laparoscopic surgery(group B)and 33 cases with glove approach single port laparoscopic surgery(group C).The baseline clinical data,intraoperative and postoperative indicators of 3 groups were compared.Results:There was no significant difference in general information among 3 groups(P>0.05).The incision length of group B was significantly longer than group A and group C(P<0.05).The operation time of group B and group C was significantly longer than group A(P<0.05).The cosmetic score of group A and group C was significantly higher than group B(P<0.05).The cosmetic score of group A was significantly higher than group C(P<0.05).Conclusions:Single incision laparoscopic surgery,special approach and glove approach single port laparoscopic surgery in the treatment of benign uterine diseases possess their own advantages and disadvantages,and clinicians can choose according to the patient's condition,economic conditions and cosmetic requirements.
