摘要
目的分析实时动态四维超声产前诊断胎儿畸形的临床意义.方法选取2018年1月至2020年12月接受超声检查的疑似胎儿畸形的55例孕妇为观察对象,均接受二维超声、四维超声检查,对比检查结果.结果以胎儿出生或终止妊娠的临床结果为金标准,55名胎儿中畸形者21例.二维超声诊断18例、四维超声诊断19例.四维超声诊断敏感度、特异度、准确度、阳性预测值、阴性预测值显著高于二维超声(P均<0.05).四维超声与二维超声在面部畸形、四肢骨骼畸形、神经系统畸形、泌尿系统畸形、消化系统畸形等方面检出率无显著性差异(P均>0.05).结论实时动态四维超声产前诊断胎儿畸形的临床意义在于敏感度、特异度及准确度较高.
Objective To analyze the clinical significance of real-time dynamic four-dimensional ultrasound in prenatal diagnosis of fetal malformations.Methods Fifty-five pregnant women with suspected fetal malformations who underwent ultrasound examination from January 2018 to December 2020 were selected as observation objects.All of them underwent two-dimensional ultrasound and four-dimensional ultrasound examination,and the examination results were compared.Results Based on the clinical results of fetal birth or termination of pregnancy as the gold standard,21 of the 55 fetuses were malformed.18 cases were diagnosed by two-dimensional ultrasound and 19 cases were diagnosed by four-dimensional ultrasound.The diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value of four-dimensional ultrasound were significantly higher than those of two-dimensional ultrasound(P<0.05).Four-dimensional ultrasound and two-dimensional ultrasound showed no significant difference in the detection rates of facial deformities,limb bone deformities,nervous system malformations,urinary system malformations,and digestive system malformations(P>0.05).Conclusion The clinical significance of real-time dynamic four-dimensional ultrasound in prenatal diagnosis of fetal malformations lies in its high sensitivity,specificity and accuracy.
