从《理虚元鉴》虚劳理论试谈新冠肺炎患者恢复期的防治策略

Discussion on Prevention and Treatment Strategies of COVID-19 Patients During Convalescence Based on the "Xulao" Theory in Lixu Yuanjian

目的 挖掘中医经典《理虚元鉴》虚劳理论及防治方法,为新型冠状病毒肺炎(Corona Virus Disease 2019,COVID-19)恢复期患者核酸“复阳”的预防和后遗症状的治疗提供理论指导。方法 归纳COVID-19患者恢复期中医病因病机,探讨《理虚元鉴》虚劳理论、治疗原则、调摄方法及可能对COVID-19患者恢复期产生的积极作用。结果 COVID-19患者恢复期可归于“虚劳”范畴,为阴阳俱虚,然各有偏重,《理虚元鉴》提出了“清金保肺、培土调中、金行清化”的治疗原则,并强调“阴虚为本从肺论治,阳虚为本从脾论治”。在恢复期间应遵循“六节”、“知防”、“二护”、“三候”、“二守”、“三禁”的调摄原则。结论 《理虚元鉴》虚劳理论可能为中医药预防新冠肺炎患者“复阳”和改善康复期患者后遗症提供理论依据和方法启示。

Objective To explore the theory and prevention methods of "Xulao" in traditional Chinese medicine classic Lixu Yuanjian, and to provide theoretical guidance for the prevention of nucleic acid "return to positive" and the treatment of sequelae of COVID-19 patients in convalescence.Methods The etiology and pathogenesis of TCM in the convalescence of COVID-19 patients were summarized, and the theory of "Xulao", treatment principles, modulating methods and possible positive effects on COVID-19 patients in the convalescence were discussed. Results The convalescence period of COVID-19 patients were classified into the category of "Xulao", which was both deficiency of Yin and Yang, but each of them had its own emphasis. Lixu Yuanjian proposed the treatment principles of "clearing and protecting the lung, cultivating earth and regulating the middle, moving mental and clearing and resolving", and emphasized "Yin deficiency based on lung treatment and Yang deficiency based on spleen treatment". During recovery,the principles of "six restraint", "knowing defense", "two protections", "three Hou", "two defenses" and "three prohibitions" were followed. Conclusion The "Xulao" theory of Lixuan Yuanjian may provide theoretical basis and method inspiration for TCM prevention of COVID-19 patients nucleic acid "return to positive" and improvement of sequelae of patients in convalescence period.
