补阳还五汤化裁治疗冠心病心绞痛(气虚血瘀型)的效果及对患者炎症反应、血管内皮功能的影响

Effect of modified Buyang Huanwu decoction on angina pectoris caused by coronary heart disease(Qi deficiency and blood stasis type) and its influences on inflammatory reaction and vascular endothelial function of patients

目的 探讨补阳还五汤化裁治疗冠心病心绞痛(气虚血瘀型)的效果及对患者炎症反应、血管内皮功能的影响。方法将2019年1月至2020年12月我院收治的150例冠心病心绞痛(气虚血瘀型)患者随机分为对照组和观察组,各75例。对照组给予西医治疗,观察组在对照组基础上联合补阳还五汤化裁治疗。比较两组治疗前、后的心脏结构和功能、炎症反应、血管内皮功能指标及生活质量。结果 治疗后,两组的LVPWd、IVST均减小,LVEF、LVFS均增加,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的hs-CRP、IL-18、sICAM-1、TNF-α水平均降低,且观察组低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的ET-1、ESM-1水平均降低,NO、VEGF水平均升高,且观察组优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的SAQ评分均升高,且观察组高于对照组,差异具有统计学意义(P<0.05)。结论 补阳还五汤化裁治疗冠心病心绞痛(气虚血瘀型)可促进患者心脏结构和功能恢复,利于改善炎症反应、血管内皮功能及生活质量,值得临床推广与应用。

Objective To investigate the effect of modified Buyang Huanwu decoction on angina pectoris caused by coronary heart disease(Qi deficiency and blood stasis type) and its influences on inflammatory reaction and vascular endothelial function of patients.Methods A total of 150 patients with angina pectoris caused by coronary heart disease(Qi deficiency and blood stasis type) admitted in our hospital from January 2019 to December 2020 were randomly divided into control group and observation group,with 75 cases in each group.The control group was treated with western medicine,and the observation group was treated with modified Buyang Huanwu decoction.The cardiac structure and function,inflammatory reaction,vascular endothelial function indexes and quality of life before and after treatment were compared between the two groups.Results After treatment,LVPWd and IVST decreased in both groups,while LVEF and LVFS increased,and those in the observation group were better than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of hs-CRP,IL-18,sICAM-1 and TNF-α in both groups decreased,and those in the observation group were lower than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of ET-1 and ESM-1 decreased in both groups,while the levels of NO and VEGF increased,and those in the observation group were better than the control group,the differences were statistically significant(P<0.05).After treatment,the SAQ score in both groups increased,and that in the observation group was higher than the control group,the difference was statistically significant(P <0.05).Conclusion Modified Buyang Huanwu decoction in patients with angina pectoris caused by coronary heart disease(Qi deficiency and blood stasis type) can promote the recovery of cardiac structure and function,and improve the inflammatory reaction,vascular endothelial function and quality of life,which is worthy of clinical promotion and application.
