DR检查在夜间急诊肋骨骨折中的应用价值

The application value of DR examination in night emergen­cy of rib fracture

目的:探讨数字X线摄影(DR)在夜间急诊肋骨骨折中的应用价值。方法:将2017年8月至2020年8月急诊科夜间收治的85例拟诊肋骨骨折作为观察对象,所有患者均接受DR检查。评价成像质量,记录检查时间及检查结果,以手术结果为准,评估诊断效能。比较DR检查对不同位置、不同形态肋骨骨折中的检出率。结果:所有成像胸片中,优级片占68.24%,合格片占30.59%;DR检查平均时间(3.62±1.05)min。85例拟诊肋骨骨折患者,经DR检出78例,检出率为91.76%;以手术结果为准,DR诊断肋骨骨折的灵敏度、特异度、准确度分别为98.72%、71.43%、96.47%。DR检查在错位状肋骨骨折中的检出率为100.00%,凹陷状肋骨骨折的检出率91.30%,均明显高于裂隙状肋骨骨折的检出率75.00%(P<0.05)。DR检查在肋骨后段骨折中的显示率为96.30%,前段骨折的显示率为97.14%,均明显高于腋段骨折的显示率85.00%(P<0.05)。结论:DR检查在夜间急诊肋骨骨折中诊断价值高,具有图像质量高,检查时间短等优点。

Objective:To explore the application value of digital radiography(DR)in night emergency rib fractures.Method:85 cases of suspected rib fractures admitted at night in the emergency department from August 2017 to August 2020 were taken as observation objects,and all patients underwent DR examination.Evaluate the imaging quality,record the inspection time and inspection results,and evaluate the diagnostic efficacy based on the surgical results.To compare the detection rate of rib fractures in different positions and different shapes by DR examination.Result:Among all chest radiographs,excellent-grade radiographs accounted for 68.24%,and qualified radiographs accounted for 30.59%,The average time of DR examination was(3.62±1.05)min.85 patients with rib fractures to be diagnosed,78 cases were detected by DR,with a detection rate of 91.76%,based on the surgical results,the sensitivity,specificity,and accuracy of DR in diagnosing rib fractures were 98.72%,71.43%,and 96.47%,respectively.The detection rate of DR examination in misaligned rib fractures was 100.00%,and the detection rate of depressed rib fractures was 91.30%,which was significantly higher than the detection rate of fractured rib fractures of 75.00%(P<0.05).The display rate of DR in posterior rib fractures was 96.30%,and the display rate of anterior segment fractures was 97.14%,which was significantly higher than the display rate of axillary fractures of 85.00%(P<0.05).Conclusion:DR examination has high diagnostic value in emergency rib fractures at night,and has the advantages of high image quality and short examination time.
