密云区1278例患者抗核抗体分析研究

Analysis on Antinuclear Antibody in 1 278 Patients of Miyun Area

目的:探讨抗核抗体(ANA)在人群中的检出率及其荧光模型和滴度在自身免疫性疾病(AID)与健康人群中的临床价值。方法:2020年1月-2021年1月将北京大学密云区医院就诊的1 278例患者及78例健康者选用间接荧光免疫法进行ANA检测。分析ANA的阳性率、荧光模型为颗粒型+胞浆型的性别差异及健康人群与AID人群ANA滴度差异。结果:密云地区1 278例患者ANA阳性率为56.34%;核型中以颗粒型+胞浆型两种核型同时存在为主,其检测率为32.71%;其在ANA阳性标本中占58.06%;在ANA阳性标本中女性颗粒型+胞浆型共328例,男性颗粒型+胞浆共90例,差异具有统计学意义(P<0.01)。AID患者中ANA的阳性率为64.17%,健康者为17.95%,二者比较差异具有统计学意义(P<0.01)。在健康者中,其阳性标本滴度的中位数为1∶100,而AID患者阳性标本中滴度中位数为1∶1 000。结论:研究分析人群中以核颗粒型+胞浆型为常见核型,且女性占比高于男性。健康人群虽然存在ANA阳性,但其滴度较低,且阳性率明显低于AID患者。

Objective:To investigate the detection rate of antinuclear antibody(ANA) and clinical value of its immunofluorescence patterns and titer in autoimmune disease(AID) and healthy population.Methods: From January 2020 to January 2020,1 278patients admitted to Miyun Hospital of Peking University and 78 healthy controls were selected for ANA detection by indirect fluorescence immunoassay.The positive rate of ANA,the sex difference of immunofluorescence patterns of speckle + cytoplasmic type,and the difference of ANA titer between healthy people and AID patients were analyzed.Results: ANA positive rate of 1 278 patients in Miyun area was 56.34%.In the ANA karyotypes,speckle type + cytoplasm type were predominant,and the detection rate was 32.71%.The proportion of ANA positive specimens was 58.06%.In ANA positive specimens,there were 328 cases of female speckle type + cytoplasm type,and 90 cases of male speckle type + cytoplasm type,and the difference was statistically significant(P<0.01).The positive rate of ANA was 64.17% in AID patients and 17.95% in healthy controls,and the difference between the two was statistically significant(P<0.01).In the healthy controls,the median titer of positive samples was 1:100,while in the AID patients,the median titer of positive samples was 1∶1 000.Conclusion: In this study analysis population,speckle type + cytoplasmic type is the common karyotype,and the proportion of female is higher than that of male.Although ANA is positive in healthy people,the titer is lower,and the positive rate is significantly lower than that in AID patients.
