连续护理模式在老年慢性病护理中的应用效果探讨

Discussion on Application Effect of Continuous Nursing Mode in Nursing of Senile Chronic Diseases

目的:分析连续护理模式在老年慢性病护理中的应用效果。方法:选取2019年3月-2020年8月东南大学附属中大医院收治的老年慢性病患者92例,随机分为对照组和观察组,各46例。对照组给予老年慢性病常规护理,观察组给予老年慢性病连续护理。比较两组护理效果。结果:观察组护理后自我护理能力量表(ESCA)评分高于对照组,差异有统计学意义(P<0.05);观察组疾病控制率高于对照组,疾控复发率与再次住院率低于对照组,差异有统计学意义(P<0.05)。观察组生活质量评分及护理满意度评分均高于对照组,差异有统计学意义(P<0.05)。结论:连续护理干预可提高老年慢性病患者的自我管理能力及疾控控制率,使患者的身心保持良好状态,提升生活质量。

Objective: To analyze the application effect of continuous nursing mode in nursing of senile chronic disease. Methods:A total of 92 elderly patients with chronic diseases in Zhongda Hospital Southeast University from March 2019 to August 2020were selected and randomly divided into control group and observation group with 46 cases in each group. The control group received routine nursing of senile chronic diseases and observation group received continuous nursing of senile chronic diseases.Results: In the observation group, the exercise of self-care agency scale(ESCA) score was higher than that in control group after nursing, and the difference was statistically significant(P<0.05);the disease control rate in the observation group was higher than that in the control group, the recurrence rate of disease control and the readmission rate were lower than those in the control group,and the difference was statistically significant(P<0.05). The quality of life score and the nursing satisfaction degree were higher than those in the control group and the difference was statistically significant(P<0.05). Conclusion: Continuous nursing intervention can improve the self-management ability and disease control rate of elderly patients with chronic disease, keep the patients in good physical and mental health, and improve their quality of life.
