精细化围术期护理对腹股沟疝日间手术患儿的积极作用及其对并发症的影响

Positive Effect of Meticulous Perioperative Nursing on Children Undergoing Day Surgery for Inguinal Hernia and its Effect on Complications

目的分析精细化围术期护理对腹股沟疝日间手术患儿的积极作用及对并发症的影响。方法选取2017年2月-2019年2月于余干县妇幼保健院行日间手术治疗的85例腹股沟疝患儿,按照随机数字表法分为对照组(n=42)与观察组(n=43),对照组给予常规围术期护理,观察组应用精细化围术期护理。比较两组麻醉诱导配合程度、改良耶鲁围手术期焦虑量表(mYPAS)评分、小儿麻醉苏醒期躁动评分量表(PAED)评分、术后相关指标、手术并发症及护理满意度。结果观察组麻醉诱导配合优良率为88.37%,高于对照组的69.05%(P<0.05);麻醉诱导时,两组mYPAS评分均高于术前1天,但观察组低于对照组,且观察组PAED评分低于对照组(P<0.05);观察组术后苏醒时间、住院时间均短于对照组,且术后疼痛评分低于对照组(P<0.05);观察组并发症发生率为4.65%,低于对照组的19.05%(P<0.05);观察组护理满意度为100.00%,高于对照组的90.48%(P<0.05)。结论精细化围术期护理可提高腹股沟疝日间手术患儿的麻醉诱导配合度,缓解其焦虑及躁动情绪,缩短患儿的术后恢复时间,同时降低其术后疼痛程度及并发症风险,有利于提高护理满意度。

Objective To analyze the positive effect of meticulous perioperative nursing on children with inguinal hernia undergoing daytime operation and its influence on complications.Methods A total of 85 children with inguinal hernia who underwent daytime surgery in Qiangan Maternal and Child Health Hospital from February 2017 to February 2019 were selected and divided into control group(n=42)and observation group(n=43)according to random number table method.The control group was given routine perioperative nursing,and the observation group was given meticulous perioperative nursing.The anesthesia induction cooperation degree,modified Yale Perioperative Anxiety Scale(mYPAS)score,Pediatric Anesthesia Awakening Agitation Scale(PAED)score,postoperative related indicators,surgical complications and nursing satisfaction were compared between the two groups.Results The excellent and good rate of anesthesia induction in the observation group was 88.37%,which was higher than 69.05%in the control group(P<0.05).At anesthesia induction,the mYPAS scores of the two groups were higher than 1 day before operation,but the observation group was lower than the control group,and the PAED score of the observation group was lower than that of the control group(P<0.05).The postoperative recovery time and hospitalization time of the observation group were shorter than those of the control group,and the postoperative pain score was lower than that of the control group(P<0.05).The incidence of complications in the observation group was 4.65%,which was lower than 19.05%in the control group(P<0.05).The nursing satisfaction of the observation group was 100.00%,which was higher than 90.48%of the control group(P<0.05).Conclusion Meticulous perioperative nursing can improve the anesthesia induction coordination of children with inguinal hernia during daytime operation,relieve their anxiety and restlessness,shorten the postoperative recovery time,and reduce the postoperative pain degree and complication risk,which is conducive to improving nursing satisfaction.
