黄芪益肾颗粒对早期糖尿病肾病患者肾功能的影响

Effect of Huang Qi Yi Shen Granule on Renal Function of Patients with Early Diabetic Nephropathy

目的:观察黄芪益肾颗粒对早期糖尿病肾病的临床疗效。方法:选取2019~2020年在广东省第二中医院门诊及住院的早期糖尿病肾病患者80例,用随机数字表将患者随机分为对照组和试验组各40例。对照组予西药(厄贝沙坦和达格列净)治疗,试验组予黄芪益肾颗粒+西药联合治疗,两组疗程均为3个月。观察两组治疗前后空腹血糖、糖化血红蛋白、尿微量白蛋白排泄率、尿微量白蛋白与肌酐比值、血肌酐值等指标的变化。结果:治疗后,两组患者的UAER、ACR值均较治疗前下降(P<0.05),试验组血肌酐较治疗前明显下降(P<0.05),而对照组治疗前后的血肌酐变化不明显(P>0.05),且试验组治疗后的UAER、ACR值均低于对照组(P<0.05)。治疗后,两组患者的血糖和糖化血红蛋白值均较治疗前下降(P<0.05),且试验组治疗后的血糖和糖化血红蛋白值均低于对照组(P<0.05)。试验组总有效率高于对照组(P<0.05)。治疗期间无明显副作用出现,无退出。结论:在糖尿病肾病早期治疗过程中采用中西医结合治疗方法,应用黄芪益肾颗粒联合达格列净+厄贝沙坦较单纯运用达格列净+厄贝沙坦治疗效果好,值得临床在治疗过程中借鉴和思考。

Objective:To observe the clinical effect of Huang qi yi shen Granules on early diabetic nephropathy.Methods:A total of 80 patients with early diabetic nephropathy were selected from outpatient and hospitalized patients in our hospital in 2019~2020.The patients were randomly divided into control group and experimental group with 40 cases in each group.The control group was treated with Western Medicine(irbesartan and dagliejing),and the experimental group was treated with Huangqi Yishen granule and western medicine.The course of treatment of both groups was 3 months.The changes of fasting blood glucose,glycosylated hemoglobin,urinary microalbumin excretion rate,the ratio of urinary microalbumin to creatinine and blood creatinine were observed before and after treatment.Results:After treatment,the UAER and ACR values of the two groups were lower than those before treatment(P<0.05),and the blood creatinine of the experimental group was significantly lower than that before treatment(P<0.05),while the blood creatinine of the control group had no significant change before and after treatment(P>0.05),and the UAER and ACR values of the experimental group were lower than those of the control groupv.After treatment,the blood glucose and glycosylated hemoglobin values of the two groups were lower than those before treatment(P<0.05),and the blood glucose and glycosylated hemoglobin values of the experimental group were lower than those of the control group(P<0.05).The total effective rate of the experimental group was higher than that of the control group(P<0.05).There were no obvious side effects and withdrawal during the treatment.Conclusion:In the early treatment of diabetic nephropathy,the combination of Chinese and Western medicine was applied,and Huangqi Yishen granule combined with Da glitin+irbesartan was better than the treatment of Da glitin plus irbesartan.
