分化型甲状腺癌患者血清MMP-9、IL-17表达水平及其临床意义

The Clinical Significance of Serum MMP-9 and IL-17 Levels in Patients with Differentiated Thyroid Cancer

目的 探讨分化型甲状腺癌(DTC)患者血清基质金属蛋白酶-9(MMP-9)、白介素-17(IL-17)表达水平及临床意义。方法 选取甲状腺良性结节组58例、DTC组40例,健康志愿者50例(健康组),采用酶联免疫法检测血清中MMP-9、IL-17水平,并分析检测结果与患者临床参数的关系。结果 DTC组血清MMP-9、IL-17水平明显高于健康组、甲状腺良性结节组,亚组分析中,不同病理分期及淋巴结转移DTC患者均存在显著差异(各组P<0.01),且随病理分期增加,其指标呈上升趋势。经Pearson分析,血清MMP-9、IL-17水平均与DTC病理分期、淋巴转移呈正相关,各组P<0.01,MMP-9、IL-17联合检测诊断DTC的AUC为0.927 (95%CI:0.873~0.967),灵敏度与特异度85.03%、91.62%。结论 血清MMP-9、IL-17在DTC患者中高表达,且与病理分期、淋巴结转移相关联合检测MMP-9、IL-17对分化型甲状腺癌的诊断及进展的判断有一定意义。

Objective To investigate the expression levels and clinical significance of matrix metalloproteinase-9(MMP-9) and interleukin-17(IL-17) in patients with differentiated thyroid cancer(DTC). Methods 58 cases of thyroid benign nodule group,40 cases of DTC group and 50 cases of healthy volunteers(healthy group) were selected. Serum MMP-9 and IL-17 levels were detected by enzyme-linked immunoassay,and the relationship between the detection results and clinical parameters of patients was analyzed. Results The serum levels of MMP-9 and IL-17 in DTC group were significantly higher than those in healthy group and benign thyroid nodule group. In subgroup analysis,there were significant differences in DTC patients with different pathological stages and lymph node metastasis(P<0.01),and the indexes showed an increasing trend with the increase of pathological stages. Pearson analysis showed that serum MMP-9 and IL-17 levels were positively correlated with DTC pathological stage and lymphat-ic metastasis(P<0.01). The AUC of DTC diagnosed by mMP-9 and IL-17 combined detection was 0.927(95%CI :0.873-0.967). The sensitivity and specificity were 85.03% and 91.62%. Conclusion Serum MMP-9 and IL-17 are highly expressed in DTC patients,and the combined detection of MMP-9 and IL-17 related to pathological stage and lymph node metastasis is of certain significance for the diagnosis and progression of differentiated thyroid cancer.
