保留自主呼吸麻醉在肺大疱切除术中的应用

Application of Retained Autonomous Respiratory Anesthesia in Pulmonary Bullae Resection

目的:探讨保留自主呼吸麻醉在肺大疱切除术患者中的应用效果。方法:将2020年4月-2021年3月在厦门大学附属第一医院实施肺大疱切除术的84例患者根据摸球法分为观察组和对照组,每组42例。对照组采用静脉麻醉,观察组采用保留自主呼吸麻醉。比较两组临床指标、血气指标、不良反应发生率及临床疗效。结果:观察组手术时间、术后住院时间短于对照组,引流量少于对照组,苏醒时间、进食时间早于对照组(P<0.05);术后1 h,观察组动脉血氧分压(PaO;)水平比对照组高,动脉血二氧化碳分压(PaCO;)水平比对照组低(P<0.05);观察组不良反应发生率比对照组低(P<0.05);观察组总有效率比对照组高(P<0.05)。结论:保留自主呼吸麻醉应用于肺大疱切除术患者中,可加快康复进程,改善血气水平,其不良反应相对较少,应用效果良好,建议在临床中推广应用。

Objective:To explore the application of retained autonomous respiratory anesthesia in pulmonary bullae resection.Method:According to the touch ball method,84 patients who underwent pulmonary bullae resection in the First Affiliated Hospital of Xiamen University from April 2020 to March 2021 were divided into the observation group and the control group,with 42 patients in each group.Intravenous anesthesia was used in the control group and retained autonomous respiratory anesthesia was used in the observation group.The clinical indexes,blood gas indexes,incidence of adverse reactions and clinical efficacy were compared between the two groups.Result:The operation time and postoperative hospital stay in the observation group were shorter than those in the control group,the drainage volume was less than that in the control group,and the waking time and eating time were earlier than those in the control group(P<0.05).One hour after operation,the level of arterial oxygen partial pressure(PaO;)in the observation group was higher than that in the control group,and the level of arterial carbon dioxide partial pressure(PaCO;)was lower than that in the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).The total effective rate of the observation group was higher than that of the control group(P<0.05).Conclusion:Application of retained autonomous respiratory anesthesia to patients undergoing pulmonary bullae resection can speed up the rehabilitation process and improve the blood gas level.Due to relatively few adverse reactions and good therapeutic effect,it is recommended to promote its application in clinical practice.
