摘要
目的:探讨丙种球蛋白联合激素治疗小儿重症腺病毒肺炎的临床效果。方法:选取福建医科大学附属泉州第一医院2019年10月-2021年9月收治的160例重症腺病毒肺炎患儿,随机分组,分为对照组(n=80)和研究组(n=80)。对照组采用常规治疗,研究组采用常规治疗联合丙种球蛋白和激素治疗。结果:研究组治疗的总有效率高于对照组,机械通气时间、氧疗时间和住院时间均较对照组短(P<0.05);与治疗前比,治疗后两组血清TNF-α、hs-CRP水平均降低,且研究组低于对照组(P<0.05);治疗后研究组患儿免疫功能指标IgA、IgG、IgM、CD4^(+)和CD4^(+)/CD8^(+)水平较治疗前和对照组高,而CD8^(+)水平较治疗前和对照组低,且差异均有统计学意义(P<0.05)。结论:丙种球蛋白联合激素治疗可提高小儿重症腺病毒肺炎的治疗效果,抑制炎症反应,提高机体免疫功能,适合在临床实施和开展。
Objective:To investigate the clinical effect of Gamma Globulin combined with hormone in the treatment of severe adenovirus pneumonia in children.Method:A total of 160 children with severe adenovirus pneumonia treated in Quanzhou First Hospital Affiliated to Fujian Medical University from October 2019 to September 2021 were randomly divided into control group(n=80)and study group(n=80).The control group was treated with routine treatment,and the study group was treated with routine treatment combined with Gamma Globulin and hormone.Result:The total effective rate of the study group was higher than that of the control group,and the time of mechanical ventilation,oxygen therapy and hospitalization were shorter than those of the control group(P<0.05).Compared with before treatment,levels of serum TNF-α,hs-CRP after treatment in the two groups were decreased,those in the study group were lower than those in the control group(P<0.05).After treatment,the levels of IgA,IgG,IgM,CD4^(+)and CD4^(+)/CD8^(+)in the study group were higher than those before treatment and the control group,while the level of CD8^(+)was lower than that before treatment and the control group,and the differences were statistically significant(P<0.05).Conclusion:Gamma Globulin combined with hormone therapy can improve the therapeutic effect of severe adenovirus pneumonia in children,inhibit inflammatory reaction and improve immune function.It is suitable for clinical implementation and development.
