去除骨瓣减压术中不同方式处理颞肌的临床疗效对比分析

Comparative Analysis of the Clinical Efficacy of Treating Temporal Muscle in Different Ways during Removing Bone Valve Decompression

目的研究去除骨瓣减压手术中颞肌3种不同手术方式,对术后减压效果、术后脑膨出、咬合力等方面的影响。方法将我院收治的去骨瓣减压患者根据术中颞肌不同处理方式,随机分为颞肌常规缝合组、颞肌切除组、颞肌筋膜松解组。每组随机抽取20人,记录不同时间段脑膨出、颅内压、双侧咬合力差值。结果颞肌切除组术后各时间段减压效果优于较其他两组(P<0.05),术后脑膨出、咬合力无明显差异(P>0.05)。结论去骨瓣减压术中切除颞肌,可达到更好的减压效果,不影响术后脑膨出及术侧咬合力。

Objective To investigate the effect of three different surgical methods of temporal muscle on the effect of postoperative decompression,postoperative encephalocele,bite force and other aspects.Methods The boneless valve decompression patients admitted in our hospital were randomly divided into temporal muscle routine suture group,temporal muscle resection group and temporal muscle fasascial loosening group according to the different treatment methods of intraoperative temporal muscle.The 20 people were randomly selected from each group,recorded the difference in intracranial pressure,encephalocele and bilateral bite force at different time periods,and the three groups were compared.Results Unlike the other two groups(P<0.05),no significant difference in encephalele and bite force(P>0.05).Conclusion The temporal muscle resection during boneless flap decompression can achieve better decompression effect and does not affect postoperative encephalele and postoperative bite force.
