Meta-analysis of Effects of Blood Flow Restriction Training on Lower Limb Strength and Physical Capacity in the Elderly

摘要 目的研究血流限制训练对老年人下肢肌肉力量和身体行为能力的影响。方法检索中国知网、万方、维普、PubMed、Science Direct、Google Scholar、Cochrane Library、EBSCO数据库中关于血流限制训练对老年人肌肉力量和身体行为能力影响的随机对照试验,检索时限均为数据库建库至2021年9月。根据纳入标准筛选出符合要求的文献,使用PEDro量表对其进行质量评价。提取资料,使用Review Manager 5.4进行meta分析。结果共纳入12篇随机对照试验,包含350名受试者。Meta分析结果显示:与对照组相比,血流限制训练显著改善受试者下肢等速肌力(SMD=0.34,95%CI:0.02~0.65,p=0.04),显著改善受试者下肢等长肌力(SMD=0.61,95%CI:0.30~0.93,p=0.0001),显著改善受试者TUGT测试结果(SMD=-1.10,95%CI:-1.44~-0.76,p<0.00001),对受试者30s座椅起立测试结果没有显著影响(SMD=0.43,95%CI=-0.11~0.96,p=0.12)。结论血流限制训练可以改善老年人下肢肌力,可以作为老年人增加肌肉力量的措施。 Objective To study the influence of blood flow restriction training on lower limb strength and physical capacity in the elderly. Methods Randomized controlled trials on the effects of blood flow restriction training on muscle strength and physical capacity in the elderly were searched from CNQI, Wan fang, VIP, PubMed, Science Direct, Google Scholar, Cochrane Library and EBSCO databases until September 2021. According to the included criteria, the literature that met the requirements was screened out, and the PEDro scale was used for quality evaluation. After data extraction, meta-analysis was performed using Review Manager 5.4. Results A total of 350 subjects were included in 12 RCTS. Meta-analysis showed that compared with the control group, exercise combined with blood flow restriction significantly improved the isokinetic muscle strength of lower limbs(SMD=0.34, 95%CI:0.02-0.65, p=0.04),and the isometric muscle strength of lower limbs(SMD=0.61, 95%CI:0.30-0.93, p<0.000 1), significantly improved TUGT test results(SMD=-1.10, 95%CI:-1.44-0.76, p<0.00001), but had no significant effect on Time Chair Rise test results(SMD=0.43, 95%CI:-0.11-0.96, p=0.12). Conclusion Blood flow restriction training can improve muscle strength of lower limb in the elderly, which can be used as a measure to improve muscle strength in the elderly.
