摘要
目的:通过分析总结肺转移瘤在CT上的表现,从而加深对肺转移瘤的认识,提高诊断准确性。方法:收集经CT诊断并经病理或临床随访确诊为肺转移瘤的病例共396例,回顾性分析肺转移瘤病灶数目、大小、形态等情况。结果:本研究共收集到肺部转移瘤396例,其中单发38例,多发358例。转移瘤形态:结节型242例,肿块型2例,不规则型1例,粟粒型2例,混合型149例。转移瘤特征:分叶征290例,毛刺征180例,空洞75例,空气支气管征73例,晕征42例,钙化3例。转移瘤平均大小约7.00(4.64,10.95)mm。合并癌性淋巴管炎105例,胸膜转移175例,胸腔积液177例。结论:肺转移瘤主要以结节型和混合型为主,分叶征,毛刺征多见,容易合并癌性淋巴管炎、胸膜转移和胸腔积液。掌握这些特征表现,将提高肺转移瘤的诊断准确率。
Objective:To deepen the understanding and improve the diagnostic accuracy of pulmonary metastases by analyzing and summarizing their manifestations on CT. Methods:A total of 396 cases diagnosed by CT and confirmed as pulmonary metastases by pathology or clinical follow-up were collected and retrospectively analyzed for the number,size and morphology of pulmonary metastases lesions. Results:A total of 396 cases of pulmonary metastases were collected in this study,including 38 single cases and 358 multiple cases. Metastatic tumor morphology:242 cases of nodular type,2 cases of mass type,1 case of irregular type,2 cases of miliary type,and 149 cases of mixed type.Metastatic tumor characteristics:lobar sign in 290 cases,burr sign in 180 cases,cavity in 75 cases,air bronchial sign in 73 cases,halo sign in 42 cases,and calcification in 3 cases. The mean size of metastases was about 7.00(4.64,10.95)mm. 105 cases of combined carcinoma lymphangitis,175 cases of pleural metastases,177 cases of pleural effusion. Conclusion:Pulmonary metastases are mainly of nodular and mixed types,with lobar signs,burr signs are common,and easily combined with cancerous lymphangitis,pleural metastases and pleural effusion. Mastering these characteristic manifestations will improve the diagnostic accuracy of pulmonary metastases.
