早孕期母体血红蛋白水平联合血管内皮功能检测对子痫前期的预测价值

The Predictive Value of First Trimester Maternal Hemoglobin combined with Vascular Endothelial Function in Preeclampsia

目的:探讨妊娠早期母体血红蛋白(Hb)水平及血管内皮功能对子痫前期(PE)的预测价值。方法:回顾性分析2017年6月—2020年3月于我院建档及分娩的孕妇共784人,其中子痫前期孕妇60人。记录孕妇的一般特征,Hb值,测量肱动脉血流介导的血管扩张值(FMD)等指标,分析与子痫前期的关系。结果:相比于正常孕妇,子痫前期孕妇年龄、BMI、早孕期Hb水平及FMD值差异有统计学意义(P<0.05)。FMD值及Hb值单独预测子痫前期时ROC曲线下面积分别是0.596(95%CI:0.507~0.685,P<0.05)、0.672(95%CI:0.583~0.761,P<0.05),两者联合预测曲线下面积为0.709(95%CI:0.630~0.788,P<0.05)。结论:早孕期母体血红蛋白水平联合FMD对子痫前期具有一定的预测价值。

Objective:To investigate the predictive value of maternal hemoglobin(Hb)level and vascular endothelial function in preeclampsia(PE)in the first trimester.Methods:A total of 784 pregnant women,including 60 cases of PE were retrospectively analyzed from June 2017 to March 2020.The general characteristics and Hb value of pregnant women were recorded,and flow-mediated dilation(FMD)of brachial artery was measured to analyze the relationship with PE.Methods:In pregnancies that developed PE,compared to those without PE,maternal age,FMD,Hb level and body mass index(BMI)was significantly different(P<0.05).The area under ROC curve for prediction of PE by FMD and Hb were 0.596(95%CI:0.507~0.685,P<0.05)and 0.672(95%CI:0.583~0.761,P<0.05)respectively,and the area under the combined prediction curve was 0.709(95%CI:0.630~0.788,P<0.05).Conclusion:Maternal hemoglobin level combined with FMD has predictive value for PE in the first trimester.
