生物反馈电刺激联合盆底肌锻炼对产后压力性尿失禁患者的疗效 被引量:7

Effect of Biofeedback Electrical Stimulation Combined with Pelvic Floor Muscle Exercise on Postpartum Stress Urinary Incontinence

摘要 目的探讨生物反馈电刺激联合盆底肌锻炼治疗产后压力性尿失禁患者的临床效果。方法选取2019年3月至2020年12月在郑州人民医院南部院区进行盆底康复训练的124例产后压力性尿失禁患者作为研究对象,按照随机数表法分为观察组(62例)和对照组(62例)。对照组患者接受盆底肌锻炼方式,观察组患者在此基础上接受生物反馈电刺激疗法,康复训练时间为12周。两组患者于康复训练前后采用尿动力检测仪对尿动力学指标进行检测,采用电刺激治疗仪对盆底功能指标进行检测,采用尿失禁生活质量(I-QOL)问卷对生活质量进行评价。结果经12周盆底康复训练后,两组患者最大尿流率、最大逼尿肌压和最大膀胱测定容积等尿动力学指标均有改善,并且观察组患者各指标水平高于对照组,差异有统计学意义(P<0.05);两组患者阴道收缩压和阴道静息压等盆底功能指标均有改善,并且观察组患者各指标水平高于对照组,差异有统计学意义(P<0.05);两组患者I-QOL评分提高,并且观察组患者行为限制、心理影响和社会障碍等维度水平均高于对照组,差异有统计学意义(P<0.05)。结论生物反馈电刺激联合盆底肌锻炼能够改善产后压力性尿失禁患者的尿动力学和盆底功能指标水平,可有效促进产后生活质量的提高,康复训练效果十分显著。 Objective To explore the clinical effect of biofeedback electrical stimulation combined with pelvic floor muscle exercise on patients with postpartum stress urinary incontinence.Methods A total of 124 patients with postpartum stress urinary incontinence who underwent pelvic floor rehabilitation training in the Southern District of Zhengzhou People’s Hospital from March 2019 to December 2020 were selected as the research objects,and were divided into observation group(62 cases)and control group(62 cases)by random number table method.The patients in the control group received pelvic floor muscle exercises,and the patients in the observation group received biofeedback electrical stimulation therapy on this basis.Rehabilitation training time was 12 weeks.Before and after the rehabilitation training,urodynamic testing instrument was used to detect urodynamic indicators,electrical stimulation therapy instrument was used to detect pelvic floor function indicators,and urinary incontinence quality of life(I-QOL)questionnaire was used to evaluate the quality of life.Results After 12 weeks of pelvic floor rehabilitation training,urodynamic indexes of maximum urine flow rate,maximum detrusor pressure and maximum bladder measurement volume in the two groups were improved,and the levels of the above indexes in the observation group were higher than the control group(P<0.05).The pelvic floor function indexes of vaginal systolic pressure and vaginal resting pressure in the two groups were improved,and the levels of the above indexes in the observation group were higher than the control group(P<0.05).The scores of I-QOL in the two groups were improved,ad the dimension scores of behavioral restriction,psychological impact and social barriers in the observation group were higher than the control group(P<0.05).Conclusion Biofeedback electrical stimulation combined with pelvic floor muscle exercise could improve the indexes of urodynamics and pelvic floor function of patients with postpartum stress urinary incontinence,and effectively promote the improvement of postpartum quality of life.The effect of rehabilitation training was very significant.
