腹腔镜修补术在胃十二指肠溃疡穿孔患者中的应用研究

Application of laparoscopic repair in patients with perforation of gastroduodenal ulcer

目的:探讨腹腔镜修补术效果。方法:选取胃十二指肠溃疡穿孔患者71例为研究对象,用随机数字表法分为对照组(36例)和研究组(35例),对照组患者行开腹手术,研究组患者行腹腔镜胃十二指肠溃疡穿孔修补术,对比临床指标、并发症、生活质量。结果:对照组患者出血量、切口长度及手术持续时间、排气时间、下床活动时间、在院总时间均显著高于研究组(P<0.05)。对照组患者感染、吻合口梗阻、化脓肿胀、残端瘘发生率显著高于研究组(P<0.05)。对照组患者躯体疼痛、躯体功能、心理健康、社会功能、健康评分均显著低于研究组(P<0.05)。结论:经腹腔镜胃十二指肠溃疡穿孔修补术应用效果良好,可改善临床指标、炎症因子水平,减少并发症。

Objective:To explore the effect of laparoscopic repair.Methods:71 patients with perforation of gastroduodenal ulcer were selected as the research object.They were randomly divided into control group(36 cases)and study group(35 cases).The control group underwent laparotomy and the study group underwent laparoscopic repair of perforation of gastroduodenal ulcer.The clinical indicators,complications and quality of life were compared.Results:The blood loss,incision length,operation duration,exhaust,ambulation and hospital stay in the control group were significantly higher than those in the study group(P<0.05).The incidence of infection,anastomotic obstruction,suppurative swelling and stump fistula in the control group was 16.7%,which was significantly higher than that in the study group(P<0.05).The scores of physical pain,physical function,mental health,social function and health in the control group were significantly lower than those in the study group(P<0.05).Conclusion:Laparoscopic repair of gastroduodenal ulcer perforation has a good application effect,which can improve the clinical indexes,inflammatory factors and reduce complications.
