数字化导板介导下精准微创根管治疗对后牙体组织 抗折性能的影响 被引量:1

Effect of precise minimally invasive root canal therapy mediated by digital guide plate on the flexural resistance of posterior teeth tissue

摘要 目的研究数字化导板介导下精准微创根管治疗对后牙体组织抗折性能的影响。方法选取2018年1月至2021年1月于我院拔除下颌第一恒磨牙的60例(60颗牙)患者为研究对象,随机将其分为对照组和试验组,各30例。对照组进行常规开髓根管治疗,试验组进行数字化导板介导下精准微创根管治疗。比较两组患者的开髓消耗时间、根管预备时间、开髓孔面积占牙[牙合]面总面积的百分比、离体牙充填修复后的载荷力、可修复折裂发生率、生活质量评分及对治疗效果的满意度。结果试验组患者的开髓消耗时间、根管预备时间短于对照组,开髓孔面积占牙[牙合]面总面积的百分比小于对照组,差异具有统计学意义(P<0.05)。试验组患者的离体牙充填修复后的载荷力显著高于对照组,差异具有统计学意义(P<0.05)。试验组患者的可修复折裂发生率显著低于对照组,差异具有统计学意义(P<0.05)。治疗后,试验组患者的躯体功能、心理功能、社会功能、物质生活评分显著高于对照组,差异具有统计学意义(P<0.05)。试验组患者对治疗效果的满意度显著高于对照组,差异具有统计学意义(P<0.05)。结论数字化导板介导下精准微创根管治疗可提高牙齿的抗折性能,缩短开髓消耗时间、根管预备时间,缩小开髓面积,且对周围健康牙齿的影响较小,具有较强的稳定性,值得推广。 Objective To study the effect of precise minimally invasive root canal therapy mediated by digital guide plate on the flexural resistance of posterior teeth tissue.Methods A total of 60 patients(60 teeth)with mandibular first permanent molars extracted in our hospital from January 2018 to January 2021 were selected as the research objects.The patients were randomly divided into control group and experimental group,with 30 cases in each group.The control group received conventional pulpotomy root canal therapy,and the experimental group received precise minimally invasive root canal therapy mediated by digital guide plate.The pulp opening consumption time,root canal preparation time,the percentage of pulp opening area in total occlusal area,loading force of isolated teeth after filling and restoration,incidence of repairable fracture,quality of life score,satisfaction to treatment effect of the two groups were compared.Results The pulp opening consumption time and root canal preparation time in the experimental group were shorter than those in the control group,the percentage of pulp opening area in total occlusal area was smaller than that in the control group,and the differences were statistically significant(P<0.05).The loading force of isolated teeth after filling and restoration in the experimental group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The incidence of repairable fracture in the experimental group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of physical function,psychological function,social function and material life in the experimental group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).The satisfaction of patients to treatment effect in the experimental group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Precise minimally invasive root canal therapy mediated by digital guide plate can improve the flexural resistance of teeth,shorten the pulp opening consumption time and root canal preparation time,reduce the area of pulpotomy,with little impact on surrounding healthy teeth,and strong stability,which is worthy of promotion.
