超声下应变弹性成像对不同病理类型乳腺炎性病灶的诊断价值

The Value of Strain Elastography in the Differential Diagnosis of Inflammatory Lesions of the Breast with Different Pathological Types

摘要 目的评价应变弹性成像(strain elastography,SE)对不同病理类型乳腺炎性病灶的诊断价值。方法纳入2019年6月至2021年6月在锦州医科大学附属第一医院和辽阳市中心医院就诊的疑似乳腺炎患者100例,所有患者经彩色多普勒超声(color doppler uitrasonography,CDU)检查后均行SE诊断病理类型,以超声下穿刺活检或外科手术切除的组织病理诊断结果作为金标准,分析对比SE与CDU对乳腺炎病灶的诊断效能(准确度、灵敏度及特异度)。结果SE诊断乳腺炎性病灶的准确度、灵敏度和特异度分别为96.0%、95.2%、100.0%,CDU诊断乳腺炎性病灶的的准确度、灵敏度和特异度分别为80.0%、78.3%、88.2%。两种方法比较,SE诊断的准确度、灵敏度明显高于CDU诊断,P<0.05;SE诊断的特异度与CDU诊断结果比较差异无统计学意义,P>0.05。结论SE技术诊断不同病理类型的乳腺炎病灶具有较高的应用价值,为临床诊断和治疗方案制定提供了重要的依据。 Objective To evaluate the value of strain elastograph(SE)in differential diagnosis of different pathological types of breast inflammatory lesions.Methods 100 patients with suspected mastitis treated in the First Affiliated Hospital of Jinzhou Medical University and Liaoyang Central Hospital from June 2019 to June 2021 were included.All patients were diagnosed by SE for pathological analysis of lesions.All patients underwent color doppler ultrasonography(CDU)to diagnose the pathological type of SE,and the histopathological diagnosis results of ultrasound puncture biopsy or surgical resection were used as the gold standard.The diagnostic efficiency(accuracy,sensitivity and specificity)of SE and CDU for mastitis lesions and the coincidence rate of SE for different pathological types of inflammatory lesions were analyzed and compared.Results The accuracy,sensitivity and specificity of SE in diagnosing breast cancer were 96.0%,95.2%and 100.0%,respectively.The accuracy,sensitivity and specificity of CDU in diagnosis of mastitis lesions were 80.0%,78.3%and 88.2%,respectively.According to the comparison of the coincidence rate of the two methods,SE diagnosis of inflammatory lesions with different pathological types has higher coincidence rate,P<0.05,with statistical significance.Conclusion SE technique has high application value in the diagnosis of mastitis lesions of different pathological types and provides an important basis for clinical diagnosis and treatment plan formulation.
