高压氧对初发型溃疡性结肠炎患者的早期疗效观察

Observation of Early Curative Effect of Hyperbaric Oxygen on Patients with Primary Ulcerative Colitis

目的观察高压氧(hyperbaric oxygen,HBO)对初发型疡性结肠炎(ulcerative colitis,UC)患者的早期临床疗效。方法收集2019年5月至2021年5月于锦州医科大学附属第一医院入院行肠镜检查并确诊为初发型、轻中度、病变局限于左半结肠的UC患者60例,随机分为对照组(A组,n=30人)和实验组(B组,n=30人),对照组在常规治疗的基础上加用美沙拉嗪肠溶片,实验组在对照组的基础上加用HBO治疗,治疗36 d后,复查肠镜,比较两组患者的治疗总有效率和血清中白细胞介素(interleukin,IL)-2,IL-4,IL-6,IL-10、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)及Thl7细胞(CD4^(+)IL-17^(+)T细胞)的表达水平。进行统计学分析。结果实验组治疗的总有效率较对照组更高(P<0.05);实验组患者较对照组患者血清中IL-2、IL-4、IL-6细胞及Thl7细胞水平下降(P<0.05),TNF-α及IL-10水平上升(P<0.05),差异具有统计学意义。结论HBO可抑制血清中炎症因素的表达、提高免疫应答、促进肠道黏膜的修复,对初发型轻中度UC患者产生较好的治疗效果,值得进一步在临床进行推广和应用。

Objective To observe early clinical curative effect of hyperbaric oxygen(HBO)on patients with primary ulcerative colitis(UC).Methods A total of 60 UC patients admitted to the First Affiliated Hospital of Jinzhou Medical University from May 2019 to May 2021 for colonoscopy and diagnosed as having initial onset,mild to moderate lesions confined to the left colon were collected.The patients were randomly divided into control group(group A,n=30 cases)and experimental group(group B,n=30 cases).The control group was additionally treated with mesalazine enteric-coated tablet on the basis of conventional treatment,and the experimental group was additionally treated with HBO on the basis of control group.After 36 days of treatment,colonoscopy was performed to compare the total effective rate and the expression levels of IL-2,IL-4,IL-6,IL-10,tumor necrosis factor-α(TNF-α),and Thl7 cells(CD4^(+)IL-17^(+)T cells)in serum.Statistical analysis was performed.Results The total effective rate of the experimental group was higher than that of the control group(P<0.05);compared with the control group,the serum levels of IL-2,IL-4,IL-6 and Thl7 cells in the experimental group were decreased(P<0.05),while the levels of TNF-αand IL-10 were increased(P<0.05),and the differences were statistically significant.Conclusion HBO can inhibit the expression of inflammatory factors in serum,improve the immune response and promote the repair of intestinal mucosa,which has a good therapeutic effect on patients with mild to moderate UC,and is worthy of further clinical promotion and application.
