认知-存在综合性团体管理对结直肠癌化疗患者健康行为的影响

Effects of cognitive-existential comprehensive group management on healthy behaviors in patients with colorectal cancer undergoing chemotherapy

目的探讨认知-存在综合性团体管理模式对结直肠癌化疗患者自我效能、自我管理能力和应对方式的影响,以提高化疗临床效果。方法241例结直肠癌化疗患者中121例给予认知-存在综合性团体管理(观察组),余120例接受常规护理(对照组),比较2组自我效能、自我管理、心理状态(焦虑、抑郁)、应对方式。结果干预前,2组自我效能、自我管理能力、心理状态、应对方式评分差异均无统计学意义(P>0.05);干预后,2组自我效能评分升高、自我管理能力评分降低、心理状态评分降低,应对方式中面对维度评分升高、回避和屈服维度评分降低,且观察组优于对照组,差异均有统计学意义(P<0.05)。结论认知-存在综合性团体管理可有效提高结直肠癌化疗患者的自我效能与自我管理能力,改善患者焦虑、抑郁情绪,加强患者积极应对疾病的心理。

Objective To explore the effects of cognitive-existential comprehensive group management on self-efficacy,self-management ability and coping styles in patients with colorectal cancer undergoing chemotherapy so as to improve clinical effect of chemotherapy.Methods Among 241 patients with colorectal cancer undergoing chemotherapy,121 received cognitive-existential comprehensive group management(observation group),and the remaining 120 cases received routine nursing(control group).Self-efficacy,self-management,psychological status(anxiety,depression)and coping styles were compared between the two groups.Results Before intervention,there were no statistically significant differences in the scores of self-efficacy,self-management ability,psychological status and coping styles between the two groups(P>0.05).After intervention,in both groups,scores of the self-efficacy were increased,scores of the self-management ability and psychological status were decreased,scores of facing dimension in coping styles were increased,and scores of avoidance and yield dimensions in coping styles were decreased.The above indexes in the observation group were better than those in the control group,and the differences were statistically significant(P<0.05).Conclusions Cognitive-existential comprehensive group management could effectively improve self-efficacy and self-management ability of patients with colorectal cancer undergoing chemotherapy,alleviate anxiety and depression of patients and strengthen the psychology of actively coping with the disease.
