摘要
目的测量手术室(operating room,OR)噪声水平,分析噪声水平与OR工作人员焦虑和任务负荷的相关性。方法测量骨科、泌尿科和普通外科手术中不同阶段的噪声水平,第1阶段:从患者进入手术室至开始切口,第2阶段:开始切口至切口缝合完毕,第3阶段:切口缝合完毕至患者退出手术室。每台手术结束后,采用状态焦虑量表(the state anxiety inventory,S-AI)、NASA任务负荷指数(the nasa task load index,NASA-TLX)量表立即评估手术室工作人员的焦虑状态和任务负荷。结果共测量403台手术的噪声水平,3个阶段的平均噪声水平为63.00 dB(A)、68.50 dB(A)和69.32 dB(A),骨科手术3个阶段的噪声水平均高于普外科手术和泌尿外科手术,差异有统计学意义(P<0.01)。骨科医生的S-AI评分高于其他科室的医生和护士,差异有统计学意义(P<0.05)。手术室工作人员的S-AI、NASA-TLX评分与噪声水平均呈正相关关系(P<0.05)。结论OR噪声水平较高,OR工作人员的焦虑和任务负荷与噪声水平呈正相关。
Objective To measure the noise level in the operating room(OR)and to analyze its correlation with the anxiety state and workload of OR staff.Methods Noise levels at different stages in orthopedic,urological and general surgical procedures were measured:stageⅠ,from the patient entering the operating room to the start of the incision;stageⅡ,from the start of the incision to the completion of the incision suture;and stageⅢ,from the completion of the incision suture to the patient’s exit from the operating room.Immediately after each operation,the State Anxiety Inventory(S-AI)and the NASA task load index(NASA-TLX)were used to assess the anxiety state and workload of the staff in the operating room.Results Noise levels were measured in a total of 403 surgical procedures.The average noise levels of the three stages were 63.00 dB(A),68.5 dB(A)and 69.32 dB(A).The noise levels of the three stages in orthopedic surgery all were higher than those in general surgery and urological surgery,and the differences were statistically significant(P<0.01).The S-AI scores of orthopedic surgeons were higher than those of doctors and nurses in other departments,and the differences were statistically significant(P<0.05).There was a positive correlation between S-AI and NASA-TLX scores of OR staff and noise levels(P<0.05).Conclusions There was a high level of noise in OR,and the anxiety and workload of OR staff were positively correlated with noise levels.
