鼻内镜修正性手术联合中鼻甲切除治疗难治性鼻-鼻窦炎的效果观察

Observation on the effect of nasal endoscopic revision surgery combined with middle turbinate resection in the treatment of refractory rhinosinusitis

目的探讨难治性鼻-鼻窦炎患者选用鼻内镜修正性手术联合中鼻甲切除治疗的价值。方法纳入92例难治性鼻-鼻窦炎患者研究(2020年2月~2021年2月),按随机数字表分为对照组(n=46,选用内镜下开放手术治疗)、观察组(n=46,选用鼻内镜修正性手术联合中鼻甲切除治疗),统计两组临床疗效、并发症率、临床指标。结果(1)临床疗效:观察组(93.48%)高于对照组(78.26%),组间对比P<0.05。(2)并发症率:观察组(4.35%)低于对照组(19.56%),组间对比P<0.05;(3)临床指标:观察组糖精清除时间、鼻阻力、临床症状评分、疼痛评分、鼻腔鼻窦结局测试-20评分、鼻内镜检查评分低于对照组,组间对比P<0.05。结论鼻内镜修正性手术联合中鼻甲切除在难治性鼻-鼻窦炎患者治疗中效果确切,可改善其临症状、鼻腔鼻窦结局,亦可降低鼻阻力,确保治疗安全性及有效性,值得借鉴。

Objective To investigate the value of the treatment of patients with refractory rhinosinusitis using nasal endoscopic revision surgery combined with middle turbinate resection.Methods A study of 92 patients with refractory rhinosinusitis(from February 2020 to February 2021)were enrolled,and they were divided into a control group(n=46,open surgery under endoscopy)and an observation group according to a random number table(N=46,using nasal endoscopic revision surgery combined with middle turbinate resection),the clinical efficacy,complication rate,and clinical indicators of the two groups were counted.Results(1)Clinical efficacy:the observation group(93.48%)was higher than the control group(78.26%),and the comparison between the groups was P<0.05.(2)Complication rate:the observation group(4.35%)was lower than the control group(19.56%),the comparison between groups was P<0.05;(3)Clinical indicators:the observation group’s saccharin clearance time,nasal resistance,clinical symptom score,pain score,Nasal cavity and sinus outcome test-20 score,nasal endoscopy score is lower than the control group,the comparison between the groups P<0.05.Conclusion Endoscopic revision surgery combined with middle turbinate resection is effective in the treatment of patients with refractory rhino-sinusitis.It can improve the clinical symptoms,nasal cavity and sinus outcome,and can also reduce nasal resistance to ensure the safety and effectiveness of the treatment.Worth learning.
