摘要
目的 探讨实时剪切波弹性成像(SWE)技术对精索静脉曲张患者睾丸生精功能的评价价值。方法 选取做过精液分析的50例左侧精索静脉曲张患者为病例组,40例无精索静脉曲张健康男性为对照组,两组均应用SWE技术测量左侧睾丸弹性模量值。病例组分为亚临床型精索静脉曲张、精索静脉曲张I度、Ⅱ度、Ⅲ度,分析病例组与对照组、病例组各亚临床型间弹性模量值的差异。结果 病例组亚临床型与对照组及I度精索静脉曲张患者与II度精索静脉曲张患者之间的睾丸弹性模量的平均值差异均无统计学意义,亚临床型患者与I度精索静脉曲张患者及病例组II度与III度精索静脉曲张患者之间的睾丸弹性模的平均值差异有统计学意义(P<0.05)。相关性分析证实,精索静脉曲张患者睾丸弹性模量值与5级精子率(PR)呈负相关(r=-0.2952,P<0.05)。结论 SWE技术可以准确地评价睾丸组织的硬度程度,为评价睾丸生精功能提供依据。
Objective To explore the value of real-time shear wave elastography(SWE) on testicular spermatogenesis in patients with varicocele.Methods From March 2019 to August 2020, 50 patients with left varicocele were selected as the case group, and 40 healthy men without varicocele were selected as the control group. Real time shear wave elastography was performed on the testis in both groups, and the E-value of the left testis was measured. The case group was divided into subclinical varicocele, grade Ⅰ, grade Ⅱ and grade Ⅲ varicocele, and the difference of E-value was analyzed.Results There was no significant difference in the mean E-value of testis between the subclinical group and the control group, and between grade Ⅰ and Grade Ⅱ varicocele patients. There was a significant difference in the mean E-value of testis between subclinical and grade Ⅰ varicocele patients and between Grade Ⅱ and Grade Ⅲ Varicocele patients(P< 0.05). The correlation analysis showed that there was a negative correlation between the mean E-value of testis and grade 5 sperm rate(PR) in patients with varicocele(r=-0.2952, P<0.05).Conclusion SWE technique can accurately evaluate the hardness of testicular tissue and provide basis for evaluating testicular spermatogenesis function.
