度洛西汀联合体外冲击波治疗慢性非特异性腰痛疗效观察

Effect of duloxetine combined with extracorporeal shock wave therapy on chronic nonspecific low back pain

目的:观察度洛西汀联合体外冲击波治疗慢性非特异性腰痛的临床治疗效果。方法:选取2019年6月-2021年11月就诊于包头医学院第三附属医院疼痛科患者120例,根据干预措施的不同分为A、B、C三组,每组40例。A组采取体外冲击波治疗(Extracorporeal shock wave therapy,ESWT),B组采取度洛西汀疗法,C组采取度洛西汀联合体外冲击波疗法。采用模拟视觉评分(Visual analogue scale,VAS)、功能障碍指数问卷表(Oswestry Disability Index,ODI)、指地距离(Finger face to Distance,FFD)比较三组临床疗效的差异性。结果:A、B、C组在治疗后2周、4周的VAS评分、ODI评分、指地距离(FFD)评分均低于治疗前,差异具有统计学意义(P<0.05)。治疗后相同时段的VAS评分、ODI评分、指地距离(FFD)的比较,C组评分均低于A组、B组,差异具有统计学意义(P<0.05)。结论:对于慢性非特异性腰痛患者,采取度洛西汀联合体外冲击波治疗法,可明显减轻患者的腰部疼痛程度,改善腰部功能障碍,较适合临床推广。

Objective:To observe the clinical effect of duloxetine combined with extracorporeal shock wave in the treatment of chronic nonspecific low back pain.Methods:A total of 120 patients with pain in the Third Affiliated Hospital of Baotou Medical College from June 2019 to November 2021 were selected and divided into groups A,B and C according to different intervention measures,with 40 cases in each group.Group A was treated with extracorporeal shock wave therapy(ESWT),group B was treated with duloxetine,and group C was treated with duloxetine combined with extracorporeal shock wave therapy.Visual analogue scale(VAS),Oswestry Disability Index(ODI)and Finger face to distance(FFD)were used to compare the differences in clinical efficacy among the three groups.Results:The VAS score,ODI score and finger-to-ground distance(FFD)score of groups A,B and C at 2 and 4 weeks after treatment were lower than those before treatment,and the difference was statistically significant(P<0.05).Compared with VAS score,ODI score and finger-to-ground distance(FFD)at the same time after treatment,the scores in group C were lower than those in group A and group B,and the difference was statistically significant(P<0.05).Conclusion:Duloxetine combined with extracorporeal shock wave therapy can significantly reduce the degree of lumbar pain in patients with chronic nonspecific low back pain and improve lumbar dysfunction,which is more suitable for clinical promotion.
