病耻感心理干预对康复期精神分裂症患者自尊水平、应对方式及生活质量的影响

Influence of stigma psychological intervention on self-esteem,coping style and quality of life in patients with schizophrenia during rehabilitation

目的:探讨病耻感心理干预对康复期精神分裂症患者自尊水平、应对方式及生活质量的影响。方法:选取2020年5月-2021年5月肇庆市复退军人医院收治的68例康复期精神分裂症患者作为本次研究对象,随机分为对照组34例,研究组34例。对照组患者给予常规护理干预,研究组在对照组基础上实施病耻感心理干预。观察比较两组服药依从性、复发率以及干预前后自尊水平、应对方式、生活质量评分情况。结果:干预后两组自尊、社交自信、学习能力、外貌、体能等缺陷感量表(FIS)评分均高于干预前(P<0.05),且研究组高于对照组(P<0.05);干预后两组面对、回避评分均高于干预前(P<0.05),且研究组高于对照组(P<0.05),屈服评分低于干预前(P<0.05),且研究组低于对照组(P<0.05);干预后两组躯体功能、心理功能、社会功能以及物质生活状态等生活质量综合问卷(GQOL1-74)评分均高于干预前(P<0.05),且研究组高于对照组(P<0.05);研究组患者服药依从性优于对照组(P<0.05),疾病复发率低于对照组(P<0.05)。结论:病耻感心理干预能够有效改善康复期精神分裂症患者的自尊水平与生活质量,同时促进积极应对方式,进一步提高患者服药依从性,降低疾病复发率,临床应用效果较好,值得应用与推广。

Objective:To investigate the effect of stigma psychological intervention on self-esteem,coping style and quality of life in patients with schizophrenia during rehabilitation.Methods:A total of 68 patients with schizophrenia who were admitted to the Zhaoqing Veterans Hospital from May 2020 to May 2021 were selected as the subjects of this study,and were randomly divided into control group and study group,with 34 cases in each group.Patients in the control group were given routine nursing intervention,while patients in the study group were given stigma psychological intervention on the basis of the control group.The medication compliance,recurrence rate,self-esteem levels,coping styles,and quality of life scores before and after intervention between the two groups were observed and compared.Results:After the intervention,the FIS scores of the two groups of self-esteem,social confidence,learning ability,appearance,and physical fitness were significantly higher than those before the intervention(P<0.05),and those of the study group were higher than the control group(P<0.05).After the intervention,the faced,avoidance score of two groups was significantly higher than before the intervention,the yield score was significantly lower than before the intervention(P<0.05),and the scores in the study group was better compared with the control group(P<0.05).The GQOL1-74 scores such as physical,psychological,social functions and the status of material life of the two groups after the intervention were significantly higher than before the intervention(P<0.05),and those of study group was higher than the control group(P<0.05).The medication compliance of the study group was significantly better than that of the control group(P<0.05),and the disease recurrence rate was significantly lower than that of the control group(P<0.05).Conclusion:Stigma psychological intervention can effectively improve the self-esteem and quality of life of patients with schizophrenia during the rehabilitation period,promote positive coping style,further improve the medication compliance of patients,and reduce the recurrence rate of disease.The clinical application effect is good,which is worthy of application and promotion.
