适用于无创消化道重建的径向充磁吻合器的设计 被引量:2

摘要 目的:针对当前内镜下磁吻合器置入困难的问题,设计一种在消化道重建术中方便置入的自组装径向充磁吻合器。方法:该吻合器由子环、母环和“J”形管组成。其中子环和母环结构一致,均由6个直径2 mm、高2 mm的圆柱形磁体组成,且6个圆柱形磁体均为径向充磁,可通过侧侧相吸排列成线形和环形。“J”形管为柔性结构,自然状态下主体为平直管道,头部为环形凹槽。通过有限元力学分析和大鼠胃肠吻合实验验证该吻合器的可行性和有效性。结果:有限元力学分析结果表明该吻合器的线形和环形转变具有稳定性和可操作性。大鼠胃肠吻合过程中未发现吻合器解离和相关并发症,吻合口愈合良好,吻合口周长和爆破压均满足要求。结论:该吻合器不仅可以经内镜置入消化道,而且能保障吻合质量,有助于实现无创消化道重建。 Objective To design a self-assembled radially magnetized anastomosis device for GI reconstruction to address the problem of difficult placement of the magnetic anastomosis device with endoscopy.Methods An anastomosis device anastomosis device was designed composed of a master link assembly and a J-shaped pipe.All the links had the same structure involving in 6 cylindrical magnets of 2 mm in diameter and 2 mm in height,and all the 6 magnets were radially magnetized and could be arranged into a linear or toroidal shape by side to side attraction.The J-shaped pipe with a flexible structure had a straight tube as its main body and an annular groove as its head in the natural state.The feasibility and effectiveness of the anastomosis device were verified by finite element mechanical analysis and rat gastrointestinal anastomosis experiments.Results The finite element mechanical analysis showed that the anastomosis device had high stability and feasibility when deforming between linear and toroidal shapes.No anastomosis device displacement and related complications were found during the gastrointestinal anastomosis in rats,the anastomosis healed well,and the anastomotic circumference and burst pressure both met the requirements.Conclusion The anastomosis device allows for endoscopic placement into GI tract with high anastomosis quality,and helps to achieve non-invasive GI reconstruction.
