摘要
目的探讨肺癌患者放射治疗周边剂量。方法选取10例右肺癌剂量人体模型,每例分别设计静态调强(IMRT)和容积旋转调强(VMAT)计划,使用热释光测量患者甲状腺和左侧乳腺的周边剂量,对比分析两种技术的测量值;使用铅衣遮盖甲状腺区,测量屏蔽后剂量值,对比分析有、无屏蔽结果。结果VMAT技术的MU、左侧乳腺剂量、甲状腺剂量低于IMRT技术,差异有统计学意义(P<0.05)。有铅衣屏蔽后的甲状腺区和左侧乳腺采用VMAT、IMRT技术的剂量值均低于无铅衣的数值,差异有统计学意义(P<0.05)。结论临床治疗中采用VMAT技术可以降低患者的周边剂量,采用铅衣屏蔽可以进一步降低肺癌患者周边的危及器官受量。
Objective To investigate the peripheral dose of radiotherapy for patients with lung cancer.Methods Ten patients with right lung cancer were enrolled for dose mannequin,and intensity-modulated radiation therapy(IMRT)and volume rotational intensity modulated(VMAT)plans were designed for each patient respectively.The peripheral doses of thyroid and left breast of patients were measured by thermoluminescence,moreover,the measured values of the two techniques were compared and analyzed.In addition the thyroid gland area was covered with lead coat,and the dose value after shielding was measured,and the results were analyzed and compared.Results The MU dose,and the doses of thyroid and left breast in VMAT group were significantly lower than those in IMRT group(P<0.05).The measured values of VMAT and IMRT in lead coat group were significantly lower than those non-lead coat group(P<0.05).Conclusion The application of VMAT technique can decrease the peripheral dose of radiotherapy for patients with lung cancer,moreover,the application of lead coat shielding can further decrease the peripheral dose of radiotherapy.
