参苓白术散合四神丸加减联合针刺、灸法治疗脾肾阳虚型腹泻的临床效果及对胃肠激素的影响观察

Observation on the Clinical Effect of Modified Shenling Baizhu Powder and Sishen Pill Combined with Acupuncture and Moxibustion in the Treatment of Diarrhea of Spleen-kidney-yang-deficiency Syndrome and Its Influence on Gastrointestinal Hormones

目的:探究及观察参苓白术散合四神丸加减联合针刺、灸法治疗脾肾阳虚型腹泻的临床效果及对胃肠激素的影响。方法:选取2018年7月-2021年5月于厦门市中医院脾胃病科门诊及住院治疗的120例脾肾阳虚型腹泻患者为研究对象,按照随机数字表法分为对照组和观察组,每组60例。对照组采用西医治疗,观察组采用参苓白术散合四神丸加减联合针刺、灸法治疗。比较两组的临床总有效率、治疗前后症状体征积分、治疗前后餐前及餐后胃肠激素水平。结果:观察组的临床总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗前两组症状体征积分、餐前及餐后胃肠激素水平比较,差异均无统计学意义(P>0.05),治疗10、20 d后,两组症状体征积分、餐前及餐后胃肠激素水平均降低,且观察组的症状体征积分、餐前及餐后胃肠激素水平均显著低于对照组,差异均有统计学意义(P<0.05)。结论:参苓白术散合四神丸加减联合针刺、灸法治疗脾肾阳虚型腹泻的临床效果较好,可显著改善患者的胃肠激素表达水平,因此在脾肾阳虚型腹泻患者中的应用价值较高。

Objective:To investigate and observe the clinical effect of modified Shenling Baizhu Powder and Sishen Pills combined with acupuncture and moxibustion in patients with diarrhea of spleen-kidney-yangdeficiency syndrome and its influence on gastrointestinal hormones.Method:A total of 120 patients with diarrhea of spleen-kidney-yang-deficiency syndrome treated in the outpatient and inpatient Department of Spleen and Stomach of Xiamen Hospital of Traditional Chinese Medicine from July 2018 to May 2021 were chosen as the research objects,and they were divided into control group(Western medicine treatment group)and observation group(modified Shenling Baizhu Powder and Sishen Pills combined with acupuncture and moxibustion group)by the method of random number table,with 60 cases in each group.The control group was treated with western medicine,and the observation group was treated with modified Shenling Baizhu Powder and Sishen Pills combined with acupuncture and moxibustion.The total effective rates,symptom and sign scores before and after treatment,levels of gastrointestinal hormone before and after meals before and after treatment of two groups were compared.Result:The total effective rate of observation group was significantly higher than that of control group,there was statistically significant difference(P<0.05).There were no statistically significant differences in the symptom and sign scores,levels of gastrointestinal hormone before and after meals of two groups before treatment(P>0.05),after 10 and 20 d of treatment,the symptom and sign scores,levels of gastrointestinal hormone before and after meals in the two groups decreased,and the symptom and sign scores,levels of gastrointestinal hormone before and after meals in the observation group were significantly lower than those in the control group(P<0.05).Conclusion:The clinical effect of modified Shenling Baizhu Powder and Sishen Pills combined with acupuncture and moxibustion in patients with diarrhea of spleenkidney-yang-deficiency syndrome is better,it can significantly improve the gastrointestinal hormones,so its application value in the patients with diarrhea of spleen-kidney-yang-deficiency syndrome is higher.
