膝单髁置换术治疗膝单间室骨性关节炎的效果

Effect of Knee Single Condylar Replacement in the Treatment of Knee Single Compartment Osteoarthritis

目的探讨膝单髁置换术治疗膝单间室骨性关节炎的效果。方法选取2018年1月至2020年12月郑州市第七人民医院收治的92例膝单间室骨性关节炎患者作为研究对象,按照手术方法分为观察组(46例)和对照组(46例),其中观察组接受膝单髁置换术治疗,对照组接受全膝关节置换术。比较两组患者手术相关指标、股骨-胫骨角、美国特种外科医院(HSS)评分和膝关节评分(KSS)、并发症发生率。结果观察组手术时间、住院时间短于对照组,出血量、住院费用少于对照组(P<0.05)。治疗前,两组患者股骨-胫骨角比较,差异无统计学意义(P>0.05);治疗后,对照组股骨-胫骨角低于观察组(P<0.05)。治疗后,观察组患者HSS、KSS评分高于对照组(P<0.05)。观察组术后并发症总发生率低于对照组(P<0.05)。结论同全膝关节置换术相比,膝单踝置换术不仅能改善膝单间室骨性关节炎患者的膝关节功能,减少术后并发症的发生,还能缩短手术时间、住院时间,降低住院费用。

Objective To investigate the effect of knee single condylar replacement in the treatment of knee single compartment osteoarthritis.Methods A total of 92 patients with knee single compartment osteoarthritis admitted to Zhengzhou Seventh People’s Hospital from January 2018 to December 2020 were selected and they were divided into observation group(46 cases)and control group(46 cases)by surgical methods.The observation group was treated with knee single condylar replacement,and the control group was treated with total knee arthroplasty.The surgery-related indexes,femoral-tibial angle,the Hospital for Special Surgery(HSS)score,knee society score(KSS)and the incidence rate of complications were compared.Results The operation time and hospitalization time in observation group were shorter than those in control group,and the amount of bleeding and hospitalization expenses in observation group were less than those in control group(P<0.05).Before treatment,there was no statistical difference in femoral-tibial angle between the two groups(P>0.05).After treatment,the femoral-tibial angle in control group was lower than that in observation group(P<0.05).After treatment,the scores of HSS and KSS in observation group were higher than those in control group(P<0.05).The total incidence of postoperative complications in observation group was lower than that in control group(P<0.05).Conclusion Compared with total knee arthroplasty,knee single condylar replacement can not only improve the knee function of patients with knee single compartment osteoarthritis and reduce the occurrence of postoperative complications,but also shorten the operation time,hospital stay and reduce the hospital expenses.
