乳腔镜腋窝淋巴清扫术治疗乳腺癌的临床效果及对患者免疫功能的影响

Clinical Effect of Mastoscopic Axillary Lymphadenectomy in the Treatment of Breast Cancer and the Influence on Immune Function of Patients

目的探讨乳腔镜腋窝淋巴清扫术治疗乳腺癌的临床效果及对患者免疫功能的影响。方法将我院收治的80例乳腺癌患者随机平均分为两组。观察组采用乳腔镜腋窝淋巴清扫术治疗,对照组采用常规腋窝淋巴结清扫术治疗,比较两组患者手术相关指标、免疫功能指标及术后并发症情况。结果观察组术中出血量、术后引流量均低于对照组,住院时间短于对照组(P<0.05)。术后3 d,观察组的CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)水平与术前比较差异无统计学意义(P>0.05);观察组的CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)水平均高于对照组,CD8^(+)水平低于对照组(P<0.05)。观察组术后并发症发生率为7.50%,低于对照组的32.50%(P<0.05)。结论乳腔镜腋窝淋巴清扫术治疗乳腺癌临床效果显著,可明显降低手术风险和术后并发症发生率,且对患者机体免疫功能影响较小。

Objective To explore the clinical effect of mastoscopic axillary lymphadenectomy in the treatment of breast cancer and the influence on immune function of patients.Methods 80 patients with breast cancer were randomly divided into two groups equally.The observation group was treated with mastoscopic axillary lymphadenectomy,and the control group was treated with conventional axillary lymphadenectomy.The operation related indicators,immune function indicators and postoperative complications were compared between the two groups.Results The intraoperative blood loss and postoperative drainage of the observation group were lower than those of the control group,and the hospitalization time was shorter than that of the control group(P<0.05).3 days after operation,the CD3^(+),CD4^(+) and CD4^(+)/CD8^(+) levels of the observation group had no statistical difference with those before operation(P>0.05);The CD3^(+),CD4^(+) and CD4^(+)/CD8^(+) levels of the observation group were higher than those of the control group,and the CD8^(+) level was lower than that of the control group(P<0.05).The incidence of postoperative complications in the observation group was 7.50%,lower than 32.50% in the control group(P<0.05).Conclusions Mastoscopic axillary lymphadenectomy has significant clinical effect in the treatment of breast cancer,which can significantly reduce the operative risk and incidence of postoperative complications,with less influence on immune function of patients.
