妊娠期糖尿病伴亚临床甲减对孕妇25-羟基维生素D及骨代谢指标的影响

Influence of Gestational Diabetes Mellitus Complicated with Subclinical Hypothyroidism on 25-Hydroxyvitamin D and Bone Metabolism Indicators of Pregnant Women

目的探讨妊娠期糖尿病伴亚临床甲减对孕妇25-羟基维生素D和骨代谢指标的影响。方法选取2020年6月至2021年6月在我院分娩的妊娠期糖尿病伴亚临床甲减孕妇80例作为观察组,选择同期我院健康孕妇80例作为对照组,对比两组的25-羟基维生素D水平、骨代谢指标、妊娠结局。结果观察组的25-羟基维生素D水平低于对照组,且骨钙素(OST)、Ⅰ型前胶原N端肽(PINP)及β-胶原特殊序列(β-CTX)水平均高于对照组(P<0.05)。观察组早产、低体重儿、新生儿窒息发生率均明显高于对照组(P<0.05)。结论与健康孕妇相比,妊娠期糖尿病伴亚临床甲减孕妇的25-羟基维生素D水平较低,骨代谢指标较高,同时易出现早产、新生儿窒息等问题,应予以重视,保障母婴安全。

Objective To explore the influence of gestational diabetes mellitus complicated with subclinical hypothyroidism on 25-hydroxyvitamin D and bone metabolism indicators of pregnant women.Methods 80 pregnant women with gestational diabetes mellitus complicated with subclinical hypothyroidism delivered in our hospital from June 2020 to June 2021 were selected as observation group,and 80 healthy pregnant women in our hospital at the same period were selected as control group.The 25-hydroxyvitamin D levels,bone metabolism indicators and pregnancy outcomes of the two groups were compared.Results The 25-hydroxyvitamin D level of the observation group was lower than that of the control group,and the osteocalcin(OST),type Ⅰ procollagen N-terminal peptide(PINP)andβ-collagen special sequence(β-CTX)levels were higher than those of the control group(P<0.05).The incidences of premature delivery,low birth weight infants and neonatal asphyxia in the observation group were significantly higher than those in the control group(P<0.05).Conclusions Compared with healthy pregnant women,pregnant women with gestational diabetes mellitus complicated with subclinical hypothyroidism have lower 25-hydroxyvitamin D levels and higher bone metabolism indicators,who are prone to problems such as premature delivery and neonatal asphyxia.This should be paid attention to in order to ensure the safety of mothers and babies.
