小剂量罗哌卡因复合舒芬太尼在剖宫产蛛网膜下腔麻醉中的应用效果

Application Effect of Low-Dose Ropivacaine Combined with Sufentanil in Subarachnoid Anesthesia for Cesarean Section

目的探讨小剂量罗哌卡因复合舒芬太尼在剖宫产蛛网膜下腔麻醉中的应用效果。方法选取2018年7月至2020年7月在我院行剖宫产的95例产妇,随机分为对照组(n=48)和研究组(n=47)。对照组仅采用小剂量罗哌卡因进行蛛网膜下腔麻醉,研究组采用小剂量罗哌卡因复合舒芬太尼进行蛛网膜下腔麻醉。比较两组产妇的麻醉效果、术后疼痛、新生儿Apgar评分及产后不良反应情况。结果研究组麻醉起效时间短于对照组,痛觉恢复时间及麻醉持续时间长于对照组(P<0.05)。研究组产后VAS评分低于对照组(P<0.05)。两组的新生儿Apgar评分比较差异无统计学意义(P>0.05)。研究组产后不良反应发生率为10.64%,低于对照组的27.08%(P<0.05)。结论小剂量罗哌卡因复合舒芬太尼应用于剖宫产蛛网膜下腔麻醉具有较好的镇痛效果,安全性较高,且不会对新生儿造成不良影响,值得临床推广应用。

Objective To explore the application effect of low-dose ropivacaine combined with sufentanil in subarachnoid anesthesia for cesarean section.Methods 95 puerperae undergoing cesarean section in our hospital from July 2018 to July 2020 were selected and randomly divided into control group(n=48)and study group(n=47).The control group wan given low-dose ropivacaine in subarachnoid anesthesia for cesarean section,and the study group was given low-dose ropivacaine combined with sufentanil in subarachnoid anesthesia for cesarean section.The anesthetic effect,postoperative pain,neonatal Apgar score and postpartum adverse reactions were compared between the two groups.Results The onset time of anesthesia in the study group was shorter than that in the control group,and the pain recovery time and duration of anesthesia were longer than those in the control group(P<0.05).The postpartum VAS score of the study group was lower than that of the control group(P<0.05).No statistical difference was found in the neonatal Apgar scores between the two groups(P>0.05).The incidence of postpartum adverse reactions in the study group was 10.64%,lower than 27.08% in the control group(P<0.05).Conclusions Low-dose ropivacaine combined with sufentanil in subarachnoid anesthesia for cesarean section has better analgesic effect and high safety,and has no adverse effects on newborns,which is worthy of clinical promotion and application.
