摘要
目的探讨集束化综合护理对ICU机械通气患者呼吸机相关性肺炎(VAP)发生率的影响。方法选取2019年6月至2021年6月在我院ICU行机械通气的70例患者,按入院时间顺序分为对照组(n=35)和试验组(n=35)。对照组予以常规护理,试验组在对照组基础上予以集束化综合护理,比较两组患者的机械通气时间、ICU入住时间、血气分析相关指标[动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))]及VAP发生率。结果试验组的机械通气时间、ICU入住时间短于对照组(P<0.05)。护理前,两组的PaO_(2)、SaO_(2)水平比较无统计学差异(P>0.05);护理后,试验组的PaO_(2)、SaO_(2)水平均高于对照组(P<0.05)。试验组的VAP发生率为5.71%,低于对照组的22.86%(P<0.05)。结论集束化综合护理可明显缩短ICU机械通气患者的机械通气时间和ICU入住时间,改善其血气分析相关指标,降低其VAP发生率。
Objective To explore the influence of cluster comprehensive nursing on the incidence of ventilator-associated pneumonia(VAP)in ICU patients with mechanical ventilation.Methods 70 patients with mechanical ventilation in our ICU from June 2019 to June 2021 were selected and divided into control group(n=35)and experimental group(n=35)according to the admission time.The control group was given routine nursing,and the experimental group was given cluster comprehensive nursing on the basis of the control group.The mechanical ventilation time,ICU stay time,blood gas analysis related indicators[arterial blood oxygen partial pressure(PaO_(2)),arterial blood oxygen saturation(SaO_(2))]and the incidence of VAP were compared between the two groups.Results The mechanical ventilation time and ICU stay time of the experimental group were shorter than those of the control group(P<0.05).Before nursing,no statistical difference was found in the PaO_(2) and SaO_(2) levels between the two groups(P>0.05).After nursing,the PaO_(2) and SaO_(2) levels of the experimental group were higher than those of the control group(P<0.05).The incidence of VAP in the experimental group was 5.71%,lower than 22.86% in the control group(P<0.05).Conclusions Cluster comprehensive nursing can significantly shorten the mechanical ventilation time and ICU stay time of ICU patients with mechanical ventilation,improve the blood gas analysis related indicators,and reduce the incidence of VAP.
