急性髓系白血病患者自体造血干细胞移植后的预后情况及其相关影响因素 被引量:2

Prognosis and Related Influencing Factors of Patients with Acute Myeloid Leukemia after Autologous Hematopoietic Stem Cell Transplantation

摘要 目的分析急性髓系白血病(AML)患者自体造血干细胞移植(AHSCT)后的预后情况及其影响因素。方法回顾性分析2018年2月至2019年1月我院收治的52例AML患者的临床资料,所有患者均接受AHSCT治疗,治疗后随访1年,统计其预后情况并分析影响其预后的相关因素。结果52例AML患者中,预后良好45例(86.54%),预后不良7例(13.46%)。预后不良、预后良好患者移植前状态未达到CR、LDH偏高、无维持治疗占比比较差异有统计学意义(P<0.05)。经Logistic回归分析结果显示,移植前状态未达到CR、LDH偏高、无维持治疗是导致AML患者AHSCT后预后不良的影响因素(OR>1,P<0.05)。结论移植前状态未达到CR、LDH偏高、无维持治疗是导致AML患者AHSCT后预后不良的影响因素,临床可针对上述因素制定防治方案以改善患者预后。 Objective To analyze the prognosis and influencing factors of patients with acute myeloid leukemia(AML)after autologous hematopoietic stem cell transplantation(AHSCT).Methods The clinical data of 52 AML patients admitted to our hospital from February 2018 to January 2019 were retrospectively analyzed.All patients were treated with AHSCT and followed up for 1 year.The prognosis was counted and the related factors affecting the prognosis were analyzed.Results Among 52 AML patients,45 cases(86.54%)had good prognosis,and 7 cases(13.46%)had poor prognosis.Statistical difference was found in the proportion of pre-transplantation status not reaching CR,high LDH,and no maintenance treatment between patients with poor prognosis and good prognosis.Logistic regression analysis showed that pre-transplantation status not reaching CR,high LDH,and no maintenance treatment were the influencing factors for poor prognosis of AML patients after AHSCT(OR>1,P<0.05).Conclusions Pre-transplantation status not reaching CR,high LDH,and no maintenance treatment are the influencing factors leading to the poor prognosis in AML patients after AHSCT.Clinical prevention and treatment programs can be formulated according to the above factors to improve the prognosis of patients.
