糖尿病肾病的治疗机制与藏医药的认识及研究进展

The treatment mechanism of diabetic nephropathy and the understanding and research progress of Tibetan medicine

从糖尿病肾病(diabetic nephropathy,DN)是糖尿病最严重的并发症之一,是导致终级肾功能衰竭的首要原因,目前尚缺乏逆转DN的有效药物。藏医药作为我国历史悠久、具有丰富内容的传统医学,对糖尿病及其并发症有着独特的认识和丰富的用药经验。近年来许多现代研究也表明藏医药对DN引起的肾损伤具有保护作用并逐渐阐明了发生作用的活性成分和分子机制,具体有参与葡萄糖/脂质代谢调节的信号通路,抗氧化,抗炎,抗纤维化和足细胞保护等。该文从藏医药对DN的整体认识、传统用药及现代研究机制进展进行归纳总结并分析,希望能对后续开发治疗DN高效、安全、无毒的先导药物提供相关理论依据。

Diabetic nephropathy(DN) is one of the most serious complications of diabetes mellitus and the primary cause of end-stage renal failure. Currently, there is still a lack of effective drugs to reverse DN. Tibetan medicine is a traditional medicine with a long history and rich contents in China, and it has a unique understanding of diabetes and rich experience in drug use.In recent years, many modern studies have shown that Tibetan medicine has a protective effect on kidney injury caused by DN,and gradually elucidates the bioactive compounds and molecular mechanism of the effect. Specific signaling pathways involve in glucose/lipid metabolism regulation, anti-oxidation, anti-inflammatory, anti-fibrosis and podocyte protection, etc. This paper summarizes and analyzes the overall understanding of Tibetan medicine on DN, the progress of traditional drug use and modern research mechanism, hoping to provide a relevant theoretical basis for the subsequent development of efficient, safe and non-toxic lead drugs for DN treatment.
