通过T_(1)和T_(2)值评估发育性语言障碍儿童的脑微结构改变 被引量:2

Evaluation of Brain Micro-Structure Alteration in Children with Developmental Language Disorder by T_(1) and T_(2) Values

摘要 目的 分析发育性语言障碍儿童和典型发育儿童之间语言相关脑结构的纵向弛豫时间(T_(1))值和横向弛豫时间(T_(2))值,研究发育性语言障碍儿童的脑脱水和髓鞘丢失情况,并探索弛豫时间改变与语言发育商的关系。方法 搜集25例发育性语言障碍儿童为病例组和20例典型发育儿童作为对照组,行常规MRI和合成磁共振序列Magic扫描后得到T_(1)和T_(2)地图。共测量7个白质和灰质感兴趣区的T_(1)和T_(2)值进行组间比较,并分析发育性语言障碍组感兴趣区弛豫值与语言发育商的相关性。结果 与对照组相比,病例组尾状核头T_(1)值降低,T_(2)值升高,差异具有统计学意义(P<0.05)。ROC曲线分析显示,尾状核头的T_(1)和T_(2)值诊断效能均大于0.7,两者诊断效能比较无统计学意义(P>0.05)。病例组尾状核头T;值降低与语言能力下降呈正相关(r=0.658,P<0.05)。结论 发育性语言障碍儿童脑组织存在微结构改变,尾状核头T;和T;值可为发育性语言障碍儿童诊断提供参考。 Objective There may be changes in brain microstructure in children with developmental language disorder.Our aim was to analyze the longitudinal relaxation time(T_(1)) and transverse relaxation time(T_(2)) of language-related brain structure between children with developmental language disorder and the control group to study brain dehydration and myelin loss in children with developmental language disorder, and to explore the relationship between relaxation time changes and language development quotient. Methods 25 children with developmental language disorder and 20 children with typical developmental were collected as controlsgroup.T_(1)and T_(2)maps were obtained after routine magnetic resonance imaging and synthetic magnetic resonance sequence Magic scanning.The T_(1)and T_(2)values of 7 regions of interest in white matter and gray matter were measured and compared between groups, and the correlation between relaxation and language ability in patients with developmental language disorder was analyzed. Results The T_(1)values of caudate nucleus head in the developmental language disorder group were lower than that in the control group, and the T_(1)values were higher than that in the control group, and the difference was statistically significant(P< 0.05).ROC curve analysis showed that the diagnostic efficacy of T_(1)and T_(2)values of caudate nucleus head was more than 0.7,but there was no significant difference between them(P>0.05).In the case group, the decrease of T_(1)values of caudate nucleus head were positively correlated with the decrease of language ability(r=0.658,P<0.05). Conclusion There are microstructural changes in the brain tissue of children with developmental language disorder, and the values of T_(1)and T_(2)of caudate nucleus can provide reference for the diagnosis of children with developmental language disorder.
