超声引导下平面阻滞对带状疱疹神经痛患者疼痛及炎性因子的影响分析

Effect of ultrasound-guided plane block on pain and inflammatory factors in patients with herpes zoster neuralgia

目的研究和探讨超声引导下平面阻滞对带状疱疹神经痛患者疼痛及炎性因子的影响分析。方法选择2020年1月—2021年4月在郑州市第三人民医院就诊治疗的带状疱疹神经痛患者,抽取90例,作为本次研究对象,按照治疗方式的不同分为例数相同的两组,采用常规治疗手段(伐昔洛韦联合干扰素α2b)的45例对照组,加入超声引导下平面阻滞治疗的45例观察组。比较两组带状疱疹神经痛患者临床疗效;以VAS(疼痛量表)为基准,对比两组带状疱疹神经痛患者治疗前后的相关数据;分别以SAS(焦虑量表)、SDS(抑郁量表)为基准,对比两组带状疱疹神经痛患者治疗前后的负面情绪数据;比较两组带状疱疹神经痛患者治疗前后的炎性因子数据,包括:IL-6(白介素-6)、CRP(C反应蛋白)、TNF-α(肿瘤血清因子)。结果观察组带状疱疹神经痛患者治疗后的临床疗效总有效率(93.33%)较对照组(80.00%)高,组间比较差异有统计学意义(P<0.05)。治疗前两组带状疱疹神经痛患者的VAS量表数据无差异(P>0.05);治疗后观察组VAS量表数据较对照组低,组间比较差异有统计学意义(P<0.05)。治疗前两组带状疱疹神经痛患者的SAS等负面情绪量表数据无差异(P>0.05);治疗后观察组的SAS等负面情绪量表数据较对照组低,差异有统计学意义(P<0.05)。治疗前两组带状疱疹神经痛患者的CRP、TNF-α等炎性因子数据无差异(P>0.05);治疗后观察组患者的CRP、TNF-α等炎性因子数据较对照组优,差异有统计学意义(P<0.05)。结论在带状疱疹神经痛患者采用常规治疗手段的同时联合超声引导下平面阻滞,可显著减轻患者的疼痛感及负面情绪,治疗效果较好,改善了血清因子,值得临床上借鉴。

Objective To study and explore the effect of ultrasound-guided plane block on pain and inflammatory factors in patients with herpes zoster neuralgia.Methods Totally 90 patients with herpes zoster neuralgia treated in our hospital from January 2020 to April 2021 were selected as the research object.According to different treatment methods,they were divided into two groups with the same number of cases,using conventional treatment(valaciclovir combined with interferonα2 b)45 cases in the control group and 45 cases in the observation group treated with plane block under ultrasound guidance.The clinical effects of two groups of patients with herpes zoster neuralgia were compared;Based on VAS(pain scale),the related data of patients with herpes zoster neuralgia before and after treatment were compared between the two groups;SAS(anxiety scale)and SDS(depression scale)were used as benchmarks to compare the negative emotion data of patients with herpes zoster neuralgia before and after treatment;The data of inflammatory factors before and after treatment were compared between the two groups,including IL-6(interleukin-6),CRP(C-reactive protein)and TNF-α(tumor serum factor).Results The total effective rate of herpes zoster neuralgia in the observation group(93.33%)was higher than that in the control group(80.00%)(P<0.05).There was no difference in vas between the two groups before treatment(P>0.05);After treatment,the vas scale data of the observation group was lower than that of the control group,and there was a difference between the groups(P<0.05).There was no difference in SAS and other negative emotion scale data between the two groups before treatment(P>0.05);After treatment,the data of SAS and other negative emotion scales in the observation group were lower than those in the control group(P<0.05).CRP and TNF in patients with herpes zoster neuralgia before treatment had no significent difference(P>0.05);Aafter treatment the data of other inflammatory factors were better than those in the control group(P<0.05).Conclusion Routine treatment combined with ultrasound-guided plane block in patients with herpes zoster neuralgia can significantly reduce the pain and negative emotion of patients.The treatment effect is good,and the serum factors are improved,which is worthy of clinical reference.
