妇科恶性肿瘤患者病情进展知晓程度、优逝期望水平调查及影响因素分析

Disease Progression Awareness and Good Death Expectation in Patients with Gynecological Malignant Tumors and Analysis of Influencing Factors

目的:调查妇科恶性肿瘤患者病情进展知晓程度及优逝期望水平现状,并分析对患者优逝期望水平的相关影响因素。方法:本研究为横断面研究,选择2018年1月至2021年10月期间在重庆医药高等专科学校附属第一医院妇科就诊的恶性肿瘤患者作为研究对象,分别采用一般资料调查表、病情进展知晓调查表、中文版优逝期望量表进行问卷调查,并采用单因素分析及采用多元线性回归模型分析患者病情进展知晓程度及其他相关因素对优逝期望水平的影响。结果:58例妇科恶性肿瘤患者优逝期望水平总分为(157.90±35.55)分,其中得分前3位的亚维度分别为:独立自理(10.49±1.31)分、良好的家庭关系(9.30±1.74)分、获得个人尊重(9.24±1.59)分。有19例(32.76%)患者对自身疾病进展知晓,24例(41.38%)患者对自身疾病进展位部分知晓,还有15例(25.86%)患者对自身疾病进展不知晓。对妇科恶性肿瘤患者优逝期望(GDI)总得分有影响的因素(按影响程度排序)的因素包括:与家庭成员的关系、对疾病进展的知晓程度和经济压力状况,具体表现为:与家庭成员的关系越和睦、对疾病进展的知晓程度越高、经济压力越小,患者的优逝期望水平就越高。结论:与家庭成员的关系、对疾病进展的知晓程度和经济压力状况是影响妇科恶性肿瘤患者优逝期望水平的主要因素。

Objective:To investigate the awareness of disease progression and the expectation level of good death in patients with gynecological malignant tumors,and analyze factors influencing the expectation of good death.Methods:In this cross-sectional study,cancer patients treated in the department of gynecology in the First Affiliated Hospital of Chongqing Medical College from January 2018 to October 2021 were selected as research objects.General Information Questionnaire,Disease Progress Awareness Questionnaire and Good Death Inventory(Chinese version)were used for survey.Univariate analysis and multiple linear regression model were used to analyze factors influencing patients’disease progression awareness and good death expectation.Results:The total score of excellent death expectation of 58 patients with gynecological malignant tumors was(157.90±35.55).The sub-dimensions with top 3 scores were independent self-care(10.49±1.31),good family relationship(9.30±1.74)and personal respect(9.24±1.59).19 patients(32.76%)were aware of their disease progression;24 patients(41.38%)were partially aware of their disease progression;and 15 patients(25.86%)were unaware of their disease progression.Factors affecting the total score of GDI of gynecological malignant tumor patients(according to the degree of influence)included the relationship with family members,the awareness of disease progression and economic burden.Patients with harmonious family relationship,a good awareness of the disease progression,and less economic burden had higher expectation of good death.Conclusion:Relationship with family members,awareness of disease progression and economic burden are the main factors affecting the expectation of good death of patients with gynecological malignant tumors.
